Real-World Treatment Utilization and Patterns of Adjuvant Olaparib for Early-Stage Breast Cancer: Insights from the Komodo Research Database.

Batalini Felipe F, Li Qixin Q, Hayes Jennifer F JF, Mishkin Kathryn K et al.

Background and Objectives: As adjuvant treatment for human epidermal growth factor receptor 2 (HER2)-negative early-stage breast cancer (eBC) evolves, real-world data should inform olaparib utilization, sequencing, and combination patterns in clinical practice. Methods: Using Komodo US claims data (1 January 2019-31 December 2023), we retrospectively analyzed HER2-negative eBC patients on adjuvant olaparib. Continuous insurance for 12 months pre- and ≥30 days post-index date (first olaparib prescription), with follow-up through disenrollment or study end, was required. Outcomes, including time on therapy (Kaplan-Meier median persistence), adherence (proportion of days covered), dose reduction, and combination patterns, were stratified by claims-derived hormone-receptor-positive (HR+) and triple-negative BC (TNBC) status. Results: Among 176 olaparib-treated patients (median age, 44 years), the median (95% CI) time on therapy was 11.0 (9.2, 12.2) months; 27.3% (n = 48) discontinued before 12 months without re-initiating, 19.9% experienced a dose reduction, and median adherence was 90%. Olaparib was combined with other therapies in 48.3% (n = 85) (29.6% [32/108] of TNBC and 77.9% [53/68] of HR+ patients), most often with immunotherapy in TNBC (81.3%, n = 26) and endocrine therapy in HR+ (88.7%, n = 47). Conclusions: Real-world data suggest favorable persistence and dosing patterns with adjuvant olaparib. Future research should prioritize olaparib combination regimen safety and effectiveness.

Selective Estradiol Sensitivity in 12Z Human Endometriotic Epithelial Cell Line.

Banerjee Shohini S, Chopra Esha E, Smith Ian M IM, Stroka Kimberly M KM

Estradiol (E2) is a potent estrogen molecule that plays a crucial role in regulating numerous healthy and pathophysiological processes. To model estrogen-dependent cellular activities in vitro in a physiologically meaningful way, cells chosen for experimentation should exhibit sensitivity to E2. The 12Z human endometriotic epithelial cell line is gaining attention as a model for estrogen-dependent conditions, but its functional responsiveness to E2 has not been well characterized. For the passage range in the following study, we found that E2 does not statistically significantly influence 12Z proliferation, wound closure, or single-cell migration. However, 12Zs were selectively responsive to E2 in 3D migration models where the cells were physically confined. Upregulating ESR1, the primary mediator of estrogenic action, in the 12Zs did not enhance their functional E2 sensitivity. These results highlight the importance of verifying hormone responsiveness when using 12Z cells to model estrogen-dependent processes and conditions.

From Biomarker to Trigger: Biological Calibration of Stimuli-Responsive Gels Toward Precision Periodontal Therapy.

Martinović Lovre L, Šantić Roko R, Kuliš Ena E, Martinović Dinko D et al.

Endogenous stimuli-responsive gels are designed to alter drug release or matrix behavior in periodontal sites, but their responsiveness is often demonstrated under conditions that may not represent the biological compartment encountered after placement. This comprehensive review examined whether proposed periodontal triggers have been biologically calibrated. Targeted searches of PubMed/MEDLINE, publisher websites, and backward and forward citations identified human studies measuring periodontal pH, proteases, redox conditions, oxygen tension, microbial metabolites, and glucose, together with gel studies using the same cues. Human pH measurements ranged from mildly acidic to alkaline and did not support a universal activation window. Protease-responsive systems had the strongest mechanistic rationale, although recombinant-enzyme testing was not equivalent to native-fluid cleavage. Hydrogen peroxide-responsive gels were generally evaluated under challenge conditions above the limited direct gingival crevicular fluid evidence and without physiological antioxidant buffering. Oxygen-generating systems did not directly sense local oxygen tension. Some glucose challenges overlapped reported human GCF measurements, but calibration to defined periodontal sites and treatment conditions remained unverified. Across trigger classes, preclinical efficacy seldom isolated responsiveness from payload and carrier effects. Future studies should use compartment- and treatment-matched measurements, realistic input-output testing under flow and buffering, patient-derived fluids, and controls that isolate cue-dependent material behavior.

Short-Term Adverse Events Following Recombinant Zoster Vaccination in Older Adults in Australia: A Survey Study Comparing Dose 1 and Dose 2 and Influence of Dose Interval.

Dawes Lucy L, Nguyen Thuy T, Macartney Kristine K, Deng Lucy L

Self-reported adverse events following first and second recombinant zoster vaccination (RZV) doses in older adults (n = 41,069) collected through AusVaxSafety were analysed. Adverse event rates were higher following Dose 2 (54%; 95% CI, 53%-54%) than after Dose 1 (45%; 95% CI, 45%-46%), particularly with 3-6- and 7-9-month dose intervals, although medical review rates remained low (1.1%; 95% CI, 1.0%-1.2%), suggesting RZV is reactogenic but with no serious outcomes.

Label-Free Acoustic Wave Detection of Myelin Basic Protein for Monitoring of Neurodegenerative Disease.

Spagnolo Sandro S, Mikuličová Sára S, Davoudian Katharina K, Sontakke Kiran K et al.

Myelin basic protein (MBP) is released into cerebrospinal fluid during demyelination and is therefore a promising biomarker for the detection of neurodegenerative diseases such as multiple sclerosis. For the label-free detection of MBP, we developed an antifouling aptasensor based on thickness shear mode sensing. DNA aptamers were immobilized on gold-coated piezoelectric crystals functionalized with an engineered antifouling self-assembled monolayer based on 3-(2-mercaptoethoxy)propanoic acid (HS-MEG-COOH) combined with either 2-(2-mercaptoethoxy)ethanol (HS-MEG-OH) or dithiothreitol (DTT) as spacers. The binding of recombinant human MBP to the aptamer was confirmed with a corresponding decrease in resonance frequency in both phosphate-buffered saline (PBS) and artificial cerebrospinal fluid (aCSF). The aptasensor achieved a limit of detection of 11 ng mL-1 in PBS and 2 ng mL-1 in aCSF. PBS measurements were primarily applied to determine the aptasensor functionality, whereas experiments in aCSF evaluated MBP detection at concentrations relevant to reported levels in biological CSF. Selectivity was assessed with aptamer-free surfaces, cytochrome c, and human serum albumin (HSA) with much lower responses, while concentration-dependent MBP detection was maintained in aCSF despite the increased complexity of the matrix. The antifouling performance of the modified sensing surface was further demonstrated in aCSF by the significantly reduced frequency and dissipation responses compared to bare gold. With future validation in human cerebrospinal fluid (CSF) samples, the developed biosensor has the potential to enable sensitive, label-free detection of MBP in CSF for the identification and monitoring of neurodegenerative diseases.

Targeting the Complement C1 Proteases Using Camelid-Derived Single Domain Antibodies: Production, Characterization and Possible Application for Tissue/Fluid Analysis.

Haut Nolwenn N, Rossi Véronique V, Peliconi Julie J, Reiser Jean-Baptiste JB et al.

The complement C1r and C1s proteases initiate the classical complement pathway within the C1 complex, but emerging evidence points to additional roles in tissues, including cell-intrinsic functions linked to cancer and connective tissue disorders. Few tools are available to explore these plasma and tissue-relevant functions. We report the first development and characterization of anti-C1r and anti-C1s VHH single-domain antibodies (nanobodies). A lama was immunized with the purified human C1r2/C1s2 tetramer. After phage display selection, two anti-C1r VHHs (C1rCl1, C1rCl6) and one anti-C1s VHH (C1sCl7) were produced in E. coli and characterized by ELISA, Western blot, SPR and size-exclusion chromatography using recombinant protease fragments and proenzyme forms. Functional impact on C1s activation was assessed by cleavage assay, and tissue applicability by immunohistochemistry on human skin sections. All three VHHs bound with high affinity to both active and proenzyme forms. Epitope mapping localized C1rCl1 and C1rCl6 to the CCP2 module of C1r, and C1sCl7 to the CCP1 module of C1s. The two anti-C1r VHHs recognize distinct, non-overlapping epitopes and can bind simultaneously to C1r. C1rCl1 mildly inhibited C1s activation by C1r. The VHH pair C1rCl1/C1rCl6 enabled sandwich ELISA detection of C1r in serum, plasma, and cell culture supernatant. Immunohistochemistry on human skin confirmed clean C1r staining in keratinocytes and fibroblasts, with a dissociated tissue distribution compared to C1s. These first anti-C1r and anti-C1s nanobodies constitute useful tools for detection in biological fluids and tissue staining, with potential applications in diseases where these proteases play a pathological role.
