Turmeric-Loaded Type A Gelatin Hydrogels: In Vitro Assessment for Oral Wound Management Applications.

Gutierrez-Jimenez Yareth Idalia Ayerim YIA, Garcia-Contreras Rene R, Serrano-Diaz Paloma Netzayeli PN, Flores-Arriaga Juan Carlos JC et al.

Oral surgical wounds may benefit from local biomaterials capable of delivering bioactive compounds while remaining compatible with gingival cells. This study evaluated type A gelatin hydrogels containing turmeric powder (1.5, 3.1, or 12.5 mg per preparation) and a separate series prepared with a 25 mg/mL turmeric stock in dimethyl sulfoxide (DMSO). The hydrogels were assessed for macroscopic integrity, supernatant absorbance, MTT responses in human gingival fibroblasts (HGFs), prostaglandin E2 (PGE2), and exploratory microbial response against Staphylococcus aureus and Candida albicans. Powder-loaded matrices lost macroscopic integrity within approximately 45 min in PBS and 30 min in trypsin. Powder-loaded formulations generally produced MTT signals near control values, whereas several DMSO-stock formulations showed lower signals at later exposures. PGE2 did not show consistent dose-dependent suppression after normalization of the corresponding MTT-derived relative metabolic signal. Agar diffusion showed an approximate 2.4 mm clearance beyond the specimen edge against S. aureus for the 1.5 mg formulation and none against C. albicans; the gelatin control also showed visible clearance, and higher turmeric loadings did not produce measurable zones. Colony recovery decreased across the DMSO-stock formulations, but most plate counts were below the conventional countable range and were therefore interpreted descriptively. Overall, the findings provide an early in vitro characterization of rapidly disintegrating gelatin-turmeric hydrogels and identify matrix stability and assay limitations as major constraints that require further validation.

Comparative Quality Evaluation of Marketed Cefixime Trihydrate Tablet Brands Available in Bangladesh.

Islam Ariful A, Shanto Shahriar Alam SA, Akter Sumaiya S, Nashrah Antara A et al.

In low- and middle-income countries, antibiotic resistance is rising rapidly because of poor medication adherence, polypharmacy, and weak regulatory enforcement, while substandard and counterfeit medicines further accelerate resistance. Cefixime trihydrate, a broad-spectrum third-generation cephalosporin, is a widely used and frequently prescribed antibiotic in Bangladesh. This study evaluated quality attributes, including % drug content and % drug release, of eight commonly prescribed cefixime 200 mg tablet brands and compared them with the innovator-equivalent brand. Chromatographic separation was performed on a reversed-phase C18 column with UV detection at 282 nm (column temperature 30°C; run time 8 min), using methanol and 0.01 M KH2PO4 buffer (pH 4.5) at 1 mL/min. Dissolution studies were conducted using a USP Apparatus 2, and antimicrobial activity was assessed via the Kirby-Bauer disc diffusion method. All nine brands met USP potency standards (90%-110%) and dissolution criteria (≥ 75% drug release within 45 min). A one-way ANOVA with Dunnett's test (α = 0.05, two-tailed; critical value = 2.731) revealed significant differences between Tab. A, Tab. E, and Tab. F and the reference brand at 45 min. However, among local brands, seven out of eight (87.5%) complied with the FDA-recommended difference factor (f1) and similarity (f2) factor criteria. Zone of inhibition studies showed good antimicrobial activity across most of the brands at 5 μg disc load. Seven out of the eight local brands met the f1/f2 similarity criteria relative to the reference, indicating comparable in vitro dissolution behavior, whereas one brand (Tab. C) did not. These results emphasize the need for continuous quality surveillance to ensure therapeutic equivalence and combat antimicrobial resistance.

Divarasib as Compared with Sotorasib or Adagrasib in KRAS G12C-Mutated NSCLC.

Skoulidis Ferdinandos F, Ahn Myung-Ju MJ, Gadgeel Shirish S, Popat Sanjay S et al.

Divarasib, a next-generation, oral KRAS G12C inhibitor, is more potent and selective in vitro than the first-generation KRAS G12C inhibitors sotorasib and adagrasib. In this phase 3, international, open-label trial, patients with previously treated KRAS G12C-mutated advanced or metastatic non-small-cell lung cancer (NSCLC) were randomly assigned to receive divarasib orally at a dose of 400 mg once daily or control treatment (sotorasib orally at a dose of 960 mg once daily or adagrasib orally at a dose of 600 mg twice daily) until the occurrence of disease progression, unacceptable adverse events, loss of clinical benefit, withdrawal of consent, death, or trial termination. The primary end point was progression-free survival as assessed by blinded independent central review. Secondary end points included overall survival, confirmed overall response, duration of response, patient-reported outcomes, and safety. A total of 338 patients received divarasib (167 patients) or control treatment (171 patients; 156 received sotorasib and 15 received adagrasib). The median follow-up was 14.3 months. The median progression-free survival was 9.8 months with divarasib and 5.7 months with control treatment (hazard ratio for disease progression or death, 0.58; 95% confidence interval [CI], 0.44 to 0.76; P<0.001). The median overall survival was 17.7 months with divarasib and 12.0 months with control treatment (hazard ratio for death, 0.56; 95% CI, 0.41 to 0.78; P<0.001). Of the 328 patients who could be evaluated for response, a confirmed response occurred in 58.4% in the divarasib group and in 32.3% in the control group. The median duration of response was 11.9 months with divarasib and 6.9 months with control treatment. Fewer grade 3 or 4 treatment-related adverse events were noted with divarasib (24.6%) than with control treatment (34.5%). Among patients with previously treated KRAS G12C-mutated advanced or metastatic NSCLC, divarasib led to significantly longer progression-free and overall survival than control treatment. (Supported by F. Hoffmann-La Roche; Krascendo 1 ClinicalTrials.gov number, NCT06497556.).

A Malaysian Expert Consensus on The Identification, Evaluation, and Pharmacological Management of Diabetic Sensorimotor Polyneuropathy.

Lim Lee-Ling LL, Foo Siew-Hui SH, Abdul Ghani Rohana R, Ibrahim Luqman L et al.

Diabetic sensorimotor polyneuropathy (DSPN), the most common form of diabetic peripheral neuropathy (DPN), is a chronic complication of diabetes associated with sensory loss, foot ulceration, lower-limb amputation, reduced quality of life, and increased mortality. Despite its substantial clinical burden, DSPN remains underdiagnosed and undertreated in Malaysia. This consensus aimed to develop practical, evidence-informed recommendations for screening, diagnosis, and management of painful and non-painful presentations of DSPN tailored to the Malaysian healthcare setting. An expert panel comprising 11 endocrinologists from public and private healthcare institutions in Malaysia was convened. Draft recommendations were adapted from international and local guidelines for Malaysian clinical practice. Consensus development involved pre-meeting review and anonymous voting, followed by structured face-to-face discussion. Consensus was defined as ≥80% agreement. The panel finalized 61 consensus statements covering screening, diagnosis, sensory examination, and treatment. Recommendations emphasize annual screening for all individuals with diabetes and clinical examination to detect sensory deficits, including in individuals without painful symptoms. Assessment should include multiple bedside tests to evaluate small- and large-fiber function and loss of protective sensation. Recommended tests include the 10-g monofilament, vibration and pin-prick testing, and the Ipswich Touch Test, particularly in resource-limited settings. Assessment of painful and non-painful symptoms should complement rather than replace objective examination. Validated screening questionnaires, including the Malay-language PainDETECT may support assessment. Management includes optimization of glycemic control, lifestyle interventions, adjunctive pathogenetically oriented therapies such as alpha-lipoic acid, and evidence-based pharmacological treatments including pregabalin, gabapentin, duloxetine, and tricyclic antidepressants. Routine opioid use is discouraged. This Malaysian expert consensus provides practical, context-specific recommendations for early detection, diagnosis, and management of DSPN. By adapting international guidance to local healthcare realities, these recommendations aim to improve care consistency, reduce DSPN-related complications, and enhance patient outcomes in Malaysia.

High-dose regular insulin as a potential rescue therapy in severe aluminum phosphide poisoning: a case report.

Sedaghattalab Moslem M, Akbari Ghaidafeh G

Aluminum phosphide (rice tablet) can cause cardiocirculatory collapse and has negative cardiac inotropic effect. Positive inotropic effect of insulin on the cardiac myocytes is through calcium flux. Insulin increases the intramyocytes calcium concentration by increasing conductance of calcium through the L-type calcium channels and by increasing intramyocytes concentration of cyclic adenosine monophosphate. In addition to increasing the intramyocytes calcium level, insulin also enhances the responsiveness of the cardiac cells to calcium. High-dose insulin is a potential therapy for hypodynamic and hypotensive septic shock due to its positive inotropic qualities. In this paper, a critical aluminum phosphide poisoning patient (a 25-year-old Iranian young woman), presented with mixed severe metabolic acidosis and respiratory alkalosis, tachypnea, tachycardia, and hypotension. In contrast to our previous experiences that ingestion of aluminum phosphide tablets associated with multiple organ failure and severe shock in approximately all critical poisoning patients, early initiation of high-dose intravenous regular insulin for this patient prevented multiorgan dysfunction and death. Treatment with high-dose intravenous insulin associated with IV fluids, sodium bicarbonate, hydrocortisone, potassium, dextrose therapy and supportive care resolved completely signs and symptoms of patient after 3 days in ICU. High-dose intravenous regular insulin may be act as a rescue therapy for aluminum phosphide poisoning patients. While this single case suggests a potential benefit, further studies are required to confirm these findings.

Trazodone administration does not affect platelet concentration or function in horses.

Phillips Nicole A I NAI, Goggs Robert R, Pinn-Woodcock Toby L TL, Colbath Aimee C AC

To determine the effect of a clinically relevant dose of trazodone on platelet count and function in healthy horses. Prospective, controlled, cross-over study. Ten healthy horses. Horses were administered either 7 mg/kg trazodone orally in corn syrup or an equal amount of corn syrup (control), every 12 h for 2 weeks. After 2 weeks, horses underwent at least a 2-week washout period before being administered the alternate treatment (trazodone or control). Blood was collected before the first treatment administration (day 0) and after 2 weeks of treatment for each phase of the study. Complete blood cell counts, platelet function assays, and plasma trazodone concentrations were assessed. Data were analyzed using repeated measures ANOVA with significance set at p < .05. No differences in platelet count or tests of platelet function were observed at any time point after trazodone or control treatment. There were no adverse effects of trazodone administration appreciated on repeat physical examinations or daily ataxia scoring. No inhibitory effects of trazodone on platelet function in horses were detected. This study suggests administration of oral trazodone at 7 mg/kg prior to surgery is unlikely to impair platelet function in healthy horses.
