Understanding dementia in Filipino Americans: risk factors, research gaps, and the role of community-based participatory research.

Filipino Americans are the third-largest Asian American, Native Hawaiians, and Pacific Islander (AANHPI) group in the United States. Previous studies showed that they have high incident rates of Alzheimer's disease and related dementias (AD/ADRD) compared to other AANHPI groups. However, they are often aggregated into the "Asian" category in AD/ADRD research, masking distinct risk and mitigating factors. To date, no studies have directly explored AD/ADRD factors in Filipino Americans, a community that has unique characteristics from life experiences, immigration, historical and cultural context, and differential burden of genetic risk. In this perspective, we review known AD/ADRD risk and mitigating factors through the lens of the National Institute on Aging's Health Disparities Research Framework, spanning environmental, sociocultural, behavioral, and biological domains, and discuss their relevance to the Filipino American community. By identifying current knowledge gaps and contextualizing AD/ADRD risk and resilience through this framework, we aim to improve understanding of the complex, multifactorial pathways contributing to AD/ADRD outcomes across communities. Lastly, we discuss our experiences and strategies in the recruitment of Filipino Americans for AD/ADRD research studies, outlining challenges, successes, and future directions.

Localizing Prevention: Insights from Youth Voices on Culturally Grounded Substance Use Prevention Implementation in Windward O'ahu.

High rates of substance use among Native Hawaiian and Pacific Islander (NHPI) youth highlights the importance of culturally grounded prevention initiatives. Ho'ouna Pono (HP) is a culturally grounded substance use prevention program developed and tested with Hawai'i Island youth. Despite its efficacy, HP has yet to be implemented on other islands. This study explores the extent to which HP resonates with youth in Windward O'ahu and adaptations that may be necessary for local-level implementation. Rapid qualitative analysis was used to synthesize youth input from five focus groups (N = 22) regarding: (a) general opinions on the relatability of drug offer scenarios presented in the HP curriculum, (b) adaptations to the drug offer scenarios (i.e., what details made the story realistic or unrealistic), (c) drug offer scenarios relevant to students' school districts that were not presented in the HP curriculum, and (d) stories of drug prevention in their community. Results suggest that many drug offer scenarios presented in HP resonate with Windward O'ahu youth. Youth input elucidated potential surface-level modifications to make the drug offer scenarios more relevant for the community, such as setting, language, or peer pressure modifications. This research emphasizes the importance of meaningful adaptation for culturally grounded interventions and highlights the important question of for whom, and under what conditions culturally grounded interventions warrant additional adaptation for proximal communities.

Burnout in Embryologists Working in Assisted Reproduction: Data from a Cross-Sectional Worldwide Study.

Recent studies have highlighted the concerning prevalence of burnout among embryologists working in assisted reproduction (AR), with embryologists showing the highest levels among AR professionals. How-ever, data on burnout among these professionals across different world regions remain very limited. Therefore, this study aimed to assess burnout levels among embryologists across World Health Organization regions, with compari-sons among four regions. This was a cross-sectional study. Burnout was assessed using a worldwide online survey completed by 759 respondents from 74 countries. Participants com-pleted the 15-item Maslach Burnout Inventory-General Survey, which assesses emotional exhaustion, depersonal-ization, and lack of personal accomplishment. More than 80% of embryologists showed moderate or high levels of emotional exhaustion and de-personalization, whereas lack of personal accomplishment was present in a lower, although still substantial, propor-tion (67%). The comparison of means among Europe, Lat-in America, North America, and the Western Pacific showed only one significant difference: embryologists working in Europe had higher lack of personal accomplishment than those working in Latin America. However, significant differ-ences were found in the distribution of low, moderate, and high levels for all three burnout dimensions across world regions. Gender should be considered when studying burn-out in embryologists, as 97.8% of embryologists with high levels in all three burnout dimensions were women. Burnout is a serious global problem among embryologists working in AR. Future studies should investigate workplace stressors and preventive strategies to prevent and mitigate burnout, and should consider gen-der as an important variable.

Salmonella Outbreaks in Low- and Middle-Income Countries over the Last Decade: A Systematic Review and Meta-Analysis of Transmission Dynamics, Clinical Outcomes, and Antimicrobial Resistance.

Salmonella outbreaks persist as a public health burden in low- and middle-income countries (LMICs), yet comprehensive synthesis of their epidemiology over the past decade remains limited. Following PRISMA 2020 guidelines, we searched PubMed, Scopus, Web of Science, and Google Scholar for laboratory-confirmed Salmonella outbreaks in LMICs (1 January 2016-1 January 2026). Random-effects meta-analysis estimated pooled prevalence rates, with statistical heterogeneity quantified using I2 statistic. Given extreme heterogeneity across most outcomes (I2 > 90%), we present individual study ranges alongside pooled estimates. Twenty-four studies from 14 LMICs met inclusion criteria. Salmonella Typhi was the predominant pathogen (70.8% of outbreaks). Waterborne transmission was most frequent (37.5%), predominating in Africa (83.3%), whereas foodborne and fecal-oral routes were common in Asia. Pooled laboratory confirmation rate was 28.02% (95% CI: 12.2-52.1; observed range: 1.5-100%), varying by region: Southeast Asia 54.2% (95% CI: 4.3-96.9), Western Pacific 27.6% (95% CI: 8.0-56.9), and Africa 6.1% (95% CI: 5.3-6.9). The pooled case fatality rate was 7.0% (95% CI: 3.4-13.7; range: 3.6-10.0%). Fever (93.0%), abdominal pain (53.6%), and headache (52.0%) were most common symptoms. Pooled antimicrobial resistance reached 59.3% (95% CI: 26.6-85.4%), highest for trimethoprim-sulfamethoxazole (94.2%) and ciprofloxacin (61.3%). Extreme heterogeneity and wide confidence intervals were observed across some pooled estimates and should be interpreted with caution. Salmonella outbreaks in LMICs are characterized by high S. Typhi prevalence and predominant waterborne transmission. Elevated mortality and widespread first-line antibiotic resistance show the need for improved WASH infrastructure, enhanced real-time outbreak surveillance, and revised empiric treatment protocols.

Beyond the coach: contextual, community, and organizational predictors of coaches' perceived impact on athlete life skill development.

Youth sport is widely regarded as a context for promoting positive youth development (PYD) and life skill development; however, inequities in the youth sport system can limit coaches' perceived ability to foster these outcomes. Guided by an integrated youth sport system perspective, this study examined how contextual factors, community-level resource access, and organizational training and support were associated with coaches' perceived impact on athlete life skill development. Using data from the U.S. National Coach Survey (N = 4,964), hierarchical linear regression analyses were conducted with predictors entered in three blocks: (1) context (school vs. community setting, urbanicity, proportion of athletes experiencing poverty, proportion of athletes identifying as Black, Latino, Asian/Pacific Islander, or multiracial), (2) access to community-level resources (e.g., equipment, transportation, safe practice spaces), and (3) organizational support and training [coaching certification, life skills training, administrative support, peer support, compensation (paid vs. unpaid), and having been evaluated last season]. Context accounted for 5.4% of the variance in perceived impact. Access to community-level resources accounted for an additional 1.7% of the variance, with equipment availability emerging as a significant predictor. Organizational support and training variables accounted for the largest increase in variance (ΔR 2 = 6.5%, p < .01), with training in life skill development through sport, peer support, coaching certification, paid status, and prior evaluation associated with greater perceived impact on life skill development (R 2 = .136). Findings suggest that coaches' capacity to support life skill development can be shaped by factors within youth sport organizations and the broader youth sport context. More research is needed to understand drivers of coaching to support life skill development. Implications for practice, policy, and coach education are discussed.

Validation of serological markers of recent exposure to Plasmodium vivax in Cambodia: a longitudinal cohort and cross-sectional study.

Asymptomatic Plasmodium vivax infections undermine malaria control by sustaining transmission. This study aimed to validate an eight-marker serological panel (selected from 14 antigens) of P. vivax exposure to detect individuals likely harbouring hypnozoites in rural Cambodia, and to compare it with molecular surveillance for identifying at-risk populations in low-transmission settings. Two studies were conducted in Mondulkiri, Cambodia. In a longitudinal cohort (N = 471), participants were tested monthly for one year for P. vivax using qPCR. At 12 months, IgG responses to 14 antigens were assessed, and Random Forest models ('Cambodia-specific' and 'global') were used to classify previous infection, with PCR as the reference standard. In a cross-sectional survey (N = 3538), risk factors for sero- and PCR positivity were assessed using generalized linear mixed-effects models. PCR detected P. vivax in 29.9% (141/471) of participants over the 12-month follow-ups, while 31.8% (150/471) were classified as seropositive at month 12. Of 141 PCR-confirmed cases, 95 were seropositive (67.4%) and 46 seronegative (32.6%) under the 8-antigen panel. The 'global' model achieved an AUC of 0.81, with 83.3% specificity and 69.5% sensitivity for infections within nine months, increasing to 82% sensitivity for last-month infections. In the cross-sectional study, serology identified more positives than PCR, reflecting detection of exposure not captured by PCR. This study provides the first validation of serological markers of P. vivax exposure in Cambodia, showing that such tools can identify recent and asymptomatic infections, though sensitivity was below target.
