Clinical and Biochemical Predictors of Thyroid Autoantibody Levels in Hashimoto's Thyroiditis.

Hashimoto's thyroiditis (HT) is the most common autoimmune thyroid disease and is characterized by the presence of thyroid peroxidase (anti-TPO) and thyroglobulin (anti-Tg) antibodies. However, the clinical and biochemical factors associated with these antibodies remain incompletely understood. To evaluate the associations between anti-TPO and anti-Tg antibody levels and demographic, clinical, biochemical, and lifestyle-related factors in patients with HT. This retrospective cross-sectional study included 100 patients with HT. Clinical, biochemical, and laboratory data were retrospectively collected from medical records. Group comparisons were performed using non-parametric tests, correlations were assessed using Spearman's rank analysis, and multivariable linear regression was used to identify independent predictors of log-transformed anti-TPO and anti-Tg levels. Anti-TPO levels differed significantly according to TSH status and were independently associated with higher TSH concentrations, longer disease duration, and lower BMI. Smoking was the only independent predictor of anti-Tg levels. No significant associations were observed between thyroid autoantibody levels and age, sex, vitamin D status, or family history of thyroid disease. Anti-TPO and anti-Tg antibodies demonstrated distinct clinical associations in patients with HT. These findings support the complementary clinical value of both thyroid autoantibodies and should be confirmed in larger prospective multicenter studies.

Case Report: Balancing immunosuppression and infection-intravenous immunoglobulin in anti-HMGCR immune-mediated necrotizing myopathy complicated by severe pneumonia.

A 65-year-old male with a 3-year history of atorvastatin use presented with progressive proximal muscle weakness, dysphagia, and markedly elevated creatine kinase (2,277 U/L). Initially misdiagnosed with polymyositis, he deteriorated with severe pneumonia, septicemia, and type I respiratory failure requiring mechanical ventilation. Interleukin-6 rose from 12.83 to 610.9 pg/mL during sepsis. After confirmation of anti-HMGCR antibody positivity (18 arbitrary units [AU], cutoff >10 AU) via line blot assay, we discontinued methotrexate, maintained methylprednisolone at 40 mg, and initiated intravenous immunoglobulin (IVIG) 0.4 g/kg/day for 5 days with broad-spectrum antibiotics. Within two weeks, proximal muscle strength (MRC scale) improved from 2/5 to 4/5, IL-6 normalized to 8.56 pg/mL, and respiratory failure resolved. This case suggests that IVIG may serve as a safe immunomodulatory bridge in critically ill IMNM patients with severe infections.

Dietary Echinacea purpurea polysaccharide supplementation improves antioxidative status, intestinal immune, and structure of cecal microbiota in Mahuang chickens.

Echinacea purpurea polysaccharide (EPP), a key bioactive constituent of the natural immunostimulant E. purpurea, shows potential as a poultry feed additive. This study evaluated EPP's impact on growth performance, serum biochemistry indices, organ index, antioxidant capacity, immunity, intestinal health, and microbiota in Mahuang chickens. A total of 180 one-day-old Mahuang chicks (initial average body weight 39.10 ± 0.51 g) were randomly divided into six groups (six replicates, five chicks per replicate): a basal diet with 0 (CON), 0.1 g/kg levamisole hydrochloride (LMS), 100 mg/kg EPP (EPP100), 200 mg/kg EPP (EPP200), 400 mg/kg EPP (EPP400), and 800 mg/kg EPP (EPP800) for 42 days. The results showed that EPP did not affect serum biochemical indices and organ index, but significantly improved growth performance by increasing body weight gain (d 21) (p < 0.05). Additionally, EPP improved intestinal (duodenum, jejunum, and ileum) morphology by increasing villus height and villus height-to-crypt depth ratio, and decreasing crypt depth (d 21 and d 42) (p < 0.05). Com-pared to the CON and LMS groups, EPP significantly increased superoxide dismutase activity (p = 0.011) and total antioxidant capacity activity (p = 0.001) (d 21), elevated the levels of immunoglobulin (Ig) M (p < 0.001) and secretory IgA (p = 0.038) (d 21), and upregulated intestinal barrier-related genes (d 21 and d 42) (p < 0.05). 16S rRNA gene sequencing revealed that EPP treatment increased the Bacteroidetes/Firmicutes ratio. EPP also elevated the relative abundances of Bacteroides, Lactobacillus, Ruminococcus, Butyricicoccus, Oscillospira, and Akkermansia, while decreasing the relative abundance of Streptococcus (p < 0.05). Overall, the EPP groups outperformed the CON and LMS groups, with the high-dose groups (EPP400, EPP800) showing superior effects to the low-dose groups (EPP100, EPP200). In summary, this study confirmed that supplementing with EPP could boost growth performance, antioxidant capacity, immunity, intestinal barrier function, and cecal microbial community in Mahuang chickens without adverse effects, and the 400-800 mg/kg EPP appears to be optimal. This highlights the potential of EPP as a safe and effective alternative to synthetic immunostimulants in poultry production.

Cathepsin D as a central regulator of extracellular matrix remodeling in wound healing and chronic tissue pathologies.

Wound healing is a dynamic and multifactorial process involving coordinated interactions among extracellular matrix (ECM) components, growth factors, proteolytic enzymes, and immune and stromal cells. An essential yet often underappreciated aspect of this process is restoration of the epithelial permeability barrier, which prevents fluid loss and microbial invasion following injury. Disruption of this coordination contributes to chronic, non-healing wounds such as diabetic ulcers and venous leg ulcers, which remain major clinical challenges. Recent advances identify cathepsins, particularly cathepsin D, as key proteases that extend beyond lysosomal degradation to regulate critical events in tissue repair. Cathepsin D modulates keratinocyte proliferation, fibroblast invasion, angiogenesis, and ECM remodeling, and supports re-epithelialization through processing of structural proteins such as transglutaminase 1. Emerging evidence indicates that cathepsin D indirectly influences junctional remodeling, including tight and adherens junction dynamics, thereby facilitating permeability barrier reformation during wound closure. Its activity is regulated by oxidative stress, advanced glycation end products, inflammatory mediators, and iron metabolism - factors highly relevant to chronic wound microenvironments. Beyond proteolysis, cathepsin D shapes fibroblast and macrophage phenotypes and processes vascular endothelial growth factor (VEGF), integrating metabolic and signaling pathways essential for tissue regeneration. Collectively, these findings position cathepsin D as a mechanistic regulator and promising therapeutic target in wound biology. This review synthesizes mechanistic insights linking cathepsin D-mediated proteolysis to epithelial junction reorganization and permeability barrier recovery, and highlights emerging therapeutic strategies for improving chronic wound healing.

Recent Advances in Sustainable Yeast-Based Single-Cell Protein Production from Renewable Substrates Using Engineering Approaches.

Yeast-based single-cell protein (SCP) has emerged as a promising sustainable protein source due to its high protein content, favorable nutritional profile, and ability to utilize renewable feedstocks. The conversion of food-processing wastes, agricultural by-products, and one-carbon substrates into protein-rich biomass supports waste valorization and circular bioeconomy strategies. However, efficient utilization of these diverse feedstocks remains challenging due to variations in substrate composition and microbial metabolic capacity. Recent advances in metabolic engineering, cell wall engineering, adaptive laboratory evolution, random mutagenesis, and fermentation optimization have improved substrate utilization, biomass formation, and protein accumulation in yeasts. These developments have expanded the potential applications of yeast-based SCP in protein supplements, meat products, flavoring agents, and animal feed. This review provides a comprehensive overview of yeast species for SCP production, renewable feedstock utilization, engineering approaches for enhancing biomass and protein accumulation, and the emerging applications of yeast-based SCP in sustainable food and feed systems.

Anti-Inb alloimmunisation in pregnancy: Anaesthetic and perioperative blood management for caesarean delivery - A case report.

Anti-Inb, a rare high-frequency antigen antibody of the Indian blood group system, renders compatible banked blood virtually unavailable. A 34-week gravida 3 para 2 patient with dual alloimmunisation, anti-Inb (titre 1:128) and anti-D (titre 1:64), underwent elective caesarean delivery. Antenatal iron supplementation raised haemoglobin from 9.6 g/dL to 11.2 g/dL, enabling a preoperative autologous blood donation of 325 mL. The serial middle cerebral artery peak systolic velocity Doppler remained below 1.5 multiples of the median throughout, excluding foetal anaemia. The patient's brother was identified as the sole compatible allogeneic donor. Subarachnoid block with hyperbaric bupivacaine 9 mg achieved T5 blockade. The estimated blood loss was 900 mL due to uterine atony; stepwise uterotonic therapy was employed, and only the autologous unit was transfused. Maternal recovery was uneventful; the neonate required phototherapy but was discharged well. Antenatal haemoglobin optimisation and preoperative autologous donation are essential strategies when compatible blood is unavailable.
