Extended dosing intervals enhance the durability of a self-amplifying mRNA vaccine against pandemic H5N1 influenza: a randomized, observer-blind, dose-finding Phase I study.

Pragalos Antoinette A AA, Pacciarini Filippo F, Jin Hongfan H, Hopkins Terri T et al.

Avian influenza poses a persistent pandemic threat, yet traditional vaccines face production delays. Self-amplifying mRNA (sa-mRNA) technology offers potential advantages, including rapid development, dose sparing, and enhanced, durable immunity. We evaluated ARCT-2304, a bicistronic sa-mRNA vaccine encoding H5N1 hemagglutinin (HA) and neuraminidase (NA), in a Phase 1 randomized, observer-blind trial (NCT06602531). Healthy adults (N = 212) received two doses of ARCT-2304 (1.5, 5, or 12 mcg) or control/placebo on a 4- or 8-week schedule. Primary outcomes evaluated safety and reactogenicity (solicited/unsolicited/serious adverse events [AEs]) and immunogenicity (HA inhibition and NA enzyme-linked lectin assays 28 days post-second dose). Immunogenicity results were benchmarked using serum from a prior study supporting licensure of an H5N1 vaccine. ARCT-2304 was well-tolerated, displaying mostly mild-to-moderate, transient, dose-dependent solicited AEs, with no safety concerns identified. Two doses elicited robust HA and NA humoral responses. An 8-week interval yielded higher magnitude and greater durability than a 4-week schedule, with neutralizing and anti-NA antibodies remaining significantly elevated 6-7 months post-second dose. ARCT-2304 also induced dose-dependent, Th1-polarized T-cell responses peaking at Day 29. These findings support advancing sa-mRNA platforms for pandemic influenza preparedness.

An investigation into post-pandemic ethnic group and deprivation inequalities in influenza and COVID-19 hospitalizations.

Scott Meg M, Browning Lucy L, Roberts Tirion T, De Boer Jessica J et al.

Health system decision-makers need evidence on how modifiable factors contribute to inequalities to inform effective interventions. We examined ethnic group and deprivation inequalities in influenza and COVID-19 hospitalizations in England (2023-2024) among vaccine-eligible individuals, using causal mediation analysis to assess the roles of deprivation, vaccination uptake and comorbidities. For influenza, hospitalization odds were highest among Pakistani and Bangladeshi groups, compared with White British individuals. Deprivation mediated 26% of the total ethnic group effect for the Pakistani group and 41% for the Bangladeshi group, whilst comorbidities contributed little. Ethnic group differences in COVID-19 hospitalizations were minimal in this post-pandemic period. For both infections, the most deprived group had disproportionately high hospitalization compared to the least deprived. For influenza, vaccination uptake mediated 17%-26% of deprivation effects, whilst for COVID-19, there were no meaningful residual deprivation inequalities after including vaccination uptake and comorbidities. Vaccination uptake and deprivation mediate a substantial proportion of inequalities in hospitalization for influenza and COVID-19 in the vaccine-eligible cohort. Improving vaccination coverage in under-vaccinated groups should remain a public health priority.

Comprehensive Performance Verification of the Mindray MN2880 System in Detecting Respiratory Virus Nucleic Acids.

Chen Jinliang J, Zeng Lianying L, Huang Hao H, Chen Peisong P

To verify the clinical performance of the Mindray MN2880 system for simultaneous detection of SARS-CoV-2, influenza A virus (IAV), influenza B virus (IBV), and respiratory syncytial virus (RSV) nucleic acids. According to the CNAS-GL039:2019 and CNAS-GL037:2019 requirements for molecular diagnostic verification, we comprehensively evaluated the Mindray MN2880 multiplex respiratory virus assay across seven critical performance parameters: clinical concordance, limit of detection (LoD), repeatability, precision, cross-reactivity/interference resistance, daily QC, and contamination control. The Mindray MN2880 system demonstrated high diagnostic concordance with the reference method, with total percent agreements of 99.51% (95% CI: 99.00%-99.76%) for SARS-CoV-2, 99.93% (95% CI: 99.60%-99.99%) for IAV, and 100% for both IBV and RSV, and achieved clinically verified limits of detection (LoD: SARS-CoV-2 at 100 copies/mL, IAV/IBV/RSV at 200 copies/mL). Analytical precision was confirmed through 100% detection across negative/low/medium/high concentration panels with ≤ 3% CV for repeatability (Ct values) and ≤ 5% CV for inter-run precision. The assay exhibited 100% specificity against 6 clinically relevant pathogens, including Human Rhinovirus, parainfluenza virus, adenovirus, Mycoplasma pneumoniae, Haemophilus influenzae, and Staphylococcus aureus, with no observed cross-reactivity. Moreover, the MN2880 system demonstrated robust interference resistance (100% detection of weakly-positive samples spiked with seven interferents), stable daily QC performance (100% valid controls across 115 consecutive runs), and 100% contamination prevention. The MN2880 system satisfies key analytical benchmarks required for diagnostic use in respiratory virus multiplex testing, with performance verified against CNAS-GL039:2019 and CNAS-GL037:2019 standards.

Reoviridae-derived short trimerization domain for stabilizing homo-trimerization of vaccine immunogens and therapeutic proteins.

Wei Min M, Bu Qingfang Q, Xiao Jin J, Yuan Lunzhi L et al.

Trimerization motifs play pivotal roles in structural biology and therapeutic protein engineering. Here, we describe a short trimerization motif (rFd1303) derived from the Reoviridae family reovirus σ1 protein. Compared with the widely used T4-Foldon, the rFd1303 increased recombinant protein yields. The rFd1303-fused immunogens of SARS-CoV-2 spike and influenza hemagglutinin elicited antibody responses comparable to T4-Foldon-fused controls in mice. Using this tag, we engineered a trimeric ACE2-Ig fusion protein (TriACE2-Ig) that remained stable after 30 days at room temperature and neutralized a broad range of ACE2-utilizing sarbecoviruses, including 19 SARS-CoV-2 variants, SARS-CoV, and pangolin coronaviruses GD and GX, with higher potency than monomeric ACE2-Ig. In the hamster model challenged with various SARS-CoV-2 variants, our data demonstrated that intranasal TriACE2-Ig administration markedly reduced viral loads, virus-induced body-weight loss, lung pathology, and decreased within-cage virus transmission. These findings highlight rFd1303 as a versatile trimerization platform for vaccine and therapeutic protein development.

Interprofessional Collaboration and Exploratory Implementation Support for Community Pharmacy Influenza Vaccination in Northwestern Romania: A Cross-Sectional Survey of Pharmacists and Family Physicians.

Sabadoș Budea Adina Honoria AH, Rahotă Daniela D, Maghiar Teodor Traian TT, Ghitea Timea Claudia TC et al.

Romania introduced pharmacist-administered influenza vaccination in community pharmacies in 2022, but implementation depends on professional support, workflow capacity, and interprofessional coordination. Because the present questionnaire did not measure the full organisational construct of implementation readiness, we developed an exploratory, study-specific implementation-support score (IRS) from three attitudinal items. We conducted a cross-sectional survey in Northwestern Romania between 1 October 2025 and 30 April 2026. Of 761 questionnaires distributed, 700 were returned; 22 completely blank questionnaires were excluded, leaving 678 analysable responses (340 pharmacists and 338 family physicians). The IRS was the mean of perceived contribution to vaccination coverage, the pharmacist's role in the multidisciplinary team, and willingness to recommend pharmacy-based vaccination. Internal coherence was assessed in the development sample. Associations with IRS ≥ 4 were examined using multivariable logistic regression, and a heteroskedasticity-robust linear regression using the continuous IRS was prespecified as a sensitivity analysis. The overall IRS median was 3.00 (IQR 2.33-4.00); the mean was 3.09 ± 1.10. Pharmacists had higher scores than family physicians (median 3.67 vs. 2.67; mean 3.47 ± 1.02 vs. 2.70 ± 1.03; Mann-Whitney U = 81,200.5, p < 0.001). An IRS ≥ 4 was observed in 185 participants (27.3%), including 136 pharmacists (40.0%) and 49 family physicians (14.5%). In the adjusted logistic model, pharmacist status (aOR 4.41, 95% CI 2.66-7.33), considering direct collaboration absolutely necessary (aOR 4.22, 95% CI 1.42-12.52), annual influenza vaccination (aOR 2.98, 95% CI 1.56-5.69), and awareness of pharmacy vaccination (aOR 2.06, 95% CI 1.10-3.85) were associated with IRS ≥ 4. The continuous-score sensitivity analysis showed similar associations: pharmacist status (β 0.68, 95% CI 0.49-0.87), direct collaboration absolutely necessary (β 1.17, 95% CI 0.85-1.49), annual vaccination (β 0.51, 95% CI 0.28-0.74), and awareness (β 0.24, 95% CI 0.02-0.46). The three-item IRS showed internal coherence in this sample but represents proximal professional support rather than a validated measure of organisational implementation readiness. The findings are exploratory and associative; they should not be used as a validated institutional or policy decision threshold.
