Time-varying association of first-line bevacizumab with progression-free survival in advanced ovarian cancer: A multicenter matched cohort study.

Katić Kristina K, Čerina Pavlinović Dora D, Canjko Ivana I, Majić Ana A et al.

Randomized trials have demonstrated that first-line bevacizumab extends progression-free survival (PFS) in advanced ovarian cancer. However, its long-term effectiveness in routine clinical practice remains unclear. This study investigated the time-varying association between first-line bevacizumab and PFS in patients newly diagnosed with advanced epithelial ovarian, fallopian tube, or primary peritoneal cancer. Our retrospective cohort, drawn from four Croatian centers, included 249 eligible patients. After coarsened exact matching (CEM), 175 patients were retained: 102 received first-line bevacizumab, and 73 received no bevacizumab in either the first or second line. We used CEM-weighted, center-stratified Cox models to estimate log-time-varying and piecewise associations. A 2,000-replication full-process bootstrap determined 36-month restricted mean survival time (RMST) differences. Overall survival (OS) was a key secondary outcome. The PFS association varied significantly over time (interaction p < 0.001). Hazard ratios (HRs) with 95% confidence intervals (CIs) were: 0.12 (0.04-0.34) at 3 months, 0.69 (0.47-1.02) at 12 months and 2.81 (1.22-6.47) at 36 months. However, the piecewise HR beyond 24 months was 1.36 (0.61-3.05), influenced by sparse late risk sets. The 36-month PFS RMST difference was 3.42 months (95% CI, -0.18 to 7.40), and the OS HR was 1.15 (0.74-1.76). In a sensitivity analysis, where exposure was defined solely by first-line treatment, 217 patients were matched. Here, the 3-month PFS HR was 0.33 (0.14-0.78), the 36-month PFS RMST difference was 0.50 months (-3.02 to 4.14), and the OS HR was 1.47 (1.02-2.10). The 36-month OS RMST difference remained consistent with no significant difference. First-line bevacizumab was linked to a lower early risk of progression or death. However, this association diminished over time and was sensitive to how exposure was defined. Limitations such as calendar-period separation, sparse late follow-up data, and potential residual confounding prevent causal interpretation and do not establish definitive late benefit or harm.

AI-Based OCT Assessment of Anatomic Response During the Loading Phase of Anti-VEGF Therapy in Neovascular Age-Related Macular Degeneration.

Nassisi Marco M, Eandi Chiara Maria CM, Donà Caterina C, Pozzo Giuffrida Francesco F et al.

To compare functional and anatomic outcomes after the loading phase of intravitreal bevacizumab, aflibercept 2 mg, or faricimab in treatment-naive neovascular age-related macular degeneration (nAMD), using artificial intelligence (AI)-based optical coherence tomography (OCT) analysis. This multicenter retrospective cohort study included 224 eyes. Patients received three monthly injections of bevacizumab (n = 80), aflibercept 2 mg (n = 78), or faricimab (n = 66). Best-corrected visual acuity (BCVA) and spectral-domain OCT were assessed at baseline and week 12. AI-based segmentation quantified volumetric measurements of intraretinal fluid (IRFv), subretinal fluid (SRFv), and pigment epithelial detachment. The primary outcome was qualitative dry macula (absence of intraretinal fluid and subretinal fluid). An exploratory composite OCT biomarker (ΔCOMBO), defined as the lesser percentage reduction between IRFv and SRFv, was evaluated as a hypothesis-generating metric using receiver operating characteristic analysis. Baseline-adjusted analyses and multivariable logistic regression were performed. Descriptive within-group analyses showed improvement in BCVA and reduction in OCT parameters at week 12 in treatment groups. BCVA improvement was statistically significant in the bevacizumab and faricimab groups. In baseline-adjusted analyses, between-group differences emerged for OCT outcomes, whereas no significant between-group differences were observed for BCVA. Higher rates of dry macula were observed with faricimab (80.3%) and aflibercept 2 mg (67.9%) compared with bevacizumab (41.3%) (P < 0.001). The exploratory composite metric ΔCOMBO showed high discriminative performance (area under the curve = 0.959; 95% confidence interval, 0.907-0.999). All agents improved functional and anatomic outcomes after the loading phase in treatment-naive nAMD. Baseline-adjusted analyses showed differences in anatomic response patterns across treatments. Exploratory AI-based OCT biomarkers may help characterize short-term anatomic response; however, these findings require external validation.

A New Era of Treatment Sequencing in Platinum-Resistant Ovarian Cancer: Integrating Immunotherapy, Antibody-Drug Conjugates, and Targeted Therapies.

Mabuchi Seiji S, Kusakabe Ai A, Takimoto Yumi Y

Platinum-resistant ovarian cancer (PROC) has historically been treated with sequential non-platinum chemotherapy, with bevacizumab added when appropriate. The therapeutic landscape has expanded with positive randomized phase III data for pembrolizumab plus weekly paclitaxel with or without bevacizumab (KEYNOTE-B96), mirvetuximab soravtansine (MIRV) in FRα-high disease (MIRASOL), and relacorilant plus nab-paclitaxel (ROSELLA). Trastuzumab deruxtecan (T-DXd) has also shown substantial activity in previously treated HER2-expressing ovarian cancer, although its ovarian cohort was small, nonrandomized, and not restricted to PROC. These trials enrolled different populations and do not establish a cross-trial efficacy hierarchy. We therefore review the development of immunotherapy leading to KEYNOTE-B96, compare newer strategies with established PROC treatments, distinguish phase III evidence from regulatory approval and routine availability, and propose an evidence-informed framework for treatment selection and sequencing. The framework prioritizes current treatment opportunity while accounting for biomarker eligibility, prior treatment, residual toxicity, treatment and financial burden, patient priorities, and the feasibility of later-line therapy.

Immune-Mediated Membranoproliferative Glomerulonephritis during Pembrolizumab plus Bevacizumab Therapy in High-Grade Serous Ovarian Cancer: A Case Report.

Contreras Michel M, Reina Rossana R, González-Montero Jaime J, San Martín Tomás T et al.

Immune checkpoint inhibitors (ICIs) have transformed oncologic care but are frequently associated with immune-related adverse events (irAEs). When used in combination with anti-angiogenic agents, such as bevacizumab, the risk of renal toxicity may be amplified and requires close monitoring. We report the case of a 72-year-old woman with FIGO stage IIIC high-grade serous ovarian carcinoma treated with carboplatin, paclitaxel, bevacizumab, and pembrolizumab. After several cycles, the patient developed grade 2 hypertension, peripheral edema, nephrotic-range proteinuria, and hypoalbuminemia. Renal ultrasound was normal. Laboratory investigations revealed persistent eosinophilia, thrombocytopenia, and nephrotic syndrome. A kidney biopsy indicated immune complex-mediated membranoproliferative glomerulonephritis (MPGN) with severe podocytopathy. Comprehensive immunologic workup excluded differential diagnosis, supporting an ICI-related glomerular disease, potentially exacerbated by bevacizumab. Immunotherapy and anti-vascular endothelial growth factor therapy were discontinued. Renal function stabilized with conservative management, and corticosteroids were not required. Despite an improvement in proteinuria, the disease progressed and the patient was transitioned to a regimen of carboplatin plus pegylated liposomal doxorubicin. This case highlights the diagnostic complexity and therapeutic dilemma posed by rare renal irAEs, such as MPGN, during combination immunotherapy. Prompt recognition, kidney biopsy, and multidisciplinary management are essential. As the use of combined regimens becomes more common, awareness of glomerular complications is critical to ensure preservation of renal function and guide oncologic decisions.

Prognostic Significance of Response Dynamics in Atezolizumab plus Bevacizumab-Treated Hepatocellular Carcinoma: A Multicenter Real-World Validation Study.

Guan Renguo R, Huang Kaijun K, Zheng Zehao Z, Zhao Zhen Z et al.

Radiological response dynamics may provide prognostic information beyond conventional response categories in patients with unresectable hepatocellular carcinoma (HCC) receiving atezolizumab plus bevacizumab. Building upon findings from the IMbrave150 trial, we evaluated the prognostic significance of depth of response (DpR), duration of response (DoR), and time to response (TTR) in a real-world cohort. This retrospective multicenter study included 183 treatment-naïve patients with unresectable HCC treated with atezolizumab plus bevacizumab between November 2020 and September 2024. Tumor response was assessed according to mRECIST. Associations of DpR, DoR, and TTR with overall survival (OS) and progression-free survival (PFS) were evaluated using landmark analyses and time-dependent Cox analyses. Deeper tumor response, longer DoR, and ≥50% reductions in serologic biomarker correlated with improved survival. Patients who achieved an early response (≤2 months) tended to have less favorable survival outcomes than those with a later response. Multivariate analysis confirmed DoR (HR = 0.40; 95% CI: 0.30-0.55; p < 0.001) and DpR (HR = 0.45; 95% CI: 0.38-0.55; p < 0.001) as independent predictors for improved OS, while early response independently predicted worse OS (HR = 1.64; 95% CI: 1.26-2.15; p < 0.001). Similar trends were observed for PFS. Exploratory analyses suggested a potential association between conversion surgery and prolonged OS. DpR and DoR were independently associated with survival outcomes in patients with unresectable HCC receiving atezolizumab plus bevacizumab, extending and validating previous findings from IMbrave150 in a real-world setting. The prognostic significance of TTR and conversion surgery warrants further prospective investigation.

14-day versus 28-day trifluridine-tipiracil plus bevacizumab for refractory metastatic colorectal cancer (PRABITAS): a pragmatic, multicentre, open-label, randomised, phase 3, non-inferiority trial in Japan.

Sakakida Tomoki T, Taniguchi Hiroya H, Asayama Masako M, Suwa Yusuke Y et al.

Trifluridine-tipiracil (FTD-TPI) plus bevacizumab is the standard treatment for refractory metastatic colorectal cancer. However, the conventional schedule is frequently associated with haematological and gastrointestinal toxicity, which is clinically significant and relevant to patients. A simplified 14-day schedule has been proposed to reduce toxicity, but whether this treatment preserves survival has not been established in a randomised trial. We therefore aimed to assess whether the 14-day schedule is non-inferior to the conventional schedule for overall survival. PRABITAS was a pragmatic, multicentre, open-label, randomised, phase 3, non-inferiority trial conducted in 178 hospitals in Japan. Patients with refractory metastatic colorectal cancer were randomly assigned (1:1) using a minimisation algorithm, stratified by Eastern Cooperative Oncology Group performance status, RAS mutational status, and institution, to FTD-TPI (35 mg/m2 orally twice daily) plus bevacizumab (5 mg/kg intravenously) on either a conventional 28-day (FTD-TPI on days 1-5 and 8-12; bevacizumab on days 1 and 15) or 14-day schedule (FTD-TPI on days 1-5; bevacizumab on day 1). Eligibility criteria were broadened compared with those of conventional explanatory trials, and trial procedures were aligned with routine clinical practice. The primary endpoint was overall survival, analysed in the intention-to-treat population, with a prespecified non-inferiority margin of 1·25. Safety was assessed in all patients who received at least one dose. Missing data were not imputed. This trial is registered with the Japan Registry of Clinical Trials, jRCTs041230120, and is closed to enrolment. Between Dec 26, 2023, and May 20, 2025, we randomly assigned 972 patients, of whom 970 (99%) constituted the intention-to-treat population after two were excluded because they were randomly assigned before site approval (486 [50%] to the 28-day treatment group, 292 [60%] men and 194 [40%] women]; and 484 [50%] to the 14-day treatment group, 264 [55%] men and 220 [45%] women). Median follow-up was 17·9 months (IQR 14·3-22·6). Median overall survival was 10·3 months (95% CI 9·5-11·6) in the 28-day group and 11·3 months (10·2-12·5) in the 14-day group (stratified hazard ratio 0·93 [95% CI 0·80-1·08]). The 12-month overall survival rates were 44·0% (95% CI 39·5-48·5) in the 28-day group and 47·5% (42·9-52·0) in the 14-day group. Neutropenia grade 3 or worse was reduced in the 14-day group (13% vs 39%; absolute difference 25 percentage points [95% CI 20-31]), as was anorexia grade 2 or worse (15% vs 26%; absolute difference 11 percentage points [6-16]). Two treatment-related deaths occurred in the 14-day group and none in the 28-day group. A 14-day schedule of FTD-TPI plus bevacizumab was non-inferior to the conventional 28-day schedule for overall survival, while reducing haematological and gastrointestinal toxicity. These findings support this simplified schedule as the new standard for FTD-TPI plus bevacizumab in refractory metastatic colorectal cancer.
