An investigation into post-pandemic ethnic group and deprivation inequalities in influenza and COVID-19 hospitalizations.

Scott Meg M, Browning Lucy L, Roberts Tirion T, De Boer Jessica J et al.

Health system decision-makers need evidence on how modifiable factors contribute to inequalities to inform effective interventions. We examined ethnic group and deprivation inequalities in influenza and COVID-19 hospitalizations in England (2023-2024) among vaccine-eligible individuals, using causal mediation analysis to assess the roles of deprivation, vaccination uptake and comorbidities. For influenza, hospitalization odds were highest among Pakistani and Bangladeshi groups, compared with White British individuals. Deprivation mediated 26% of the total ethnic group effect for the Pakistani group and 41% for the Bangladeshi group, whilst comorbidities contributed little. Ethnic group differences in COVID-19 hospitalizations were minimal in this post-pandemic period. For both infections, the most deprived group had disproportionately high hospitalization compared to the least deprived. For influenza, vaccination uptake mediated 17%-26% of deprivation effects, whilst for COVID-19, there were no meaningful residual deprivation inequalities after including vaccination uptake and comorbidities. Vaccination uptake and deprivation mediate a substantial proportion of inequalities in hospitalization for influenza and COVID-19 in the vaccine-eligible cohort. Improving vaccination coverage in under-vaccinated groups should remain a public health priority.

Extended dosing intervals enhance the durability of a self-amplifying mRNA vaccine against pandemic H5N1 influenza: a randomized, observer-blind, dose-finding Phase I study.

Pragalos Antoinette A AA, Pacciarini Filippo F, Jin Hongfan H, Hopkins Terri T et al.

Avian influenza poses a persistent pandemic threat, yet traditional vaccines face production delays. Self-amplifying mRNA (sa-mRNA) technology offers potential advantages, including rapid development, dose sparing, and enhanced, durable immunity. We evaluated ARCT-2304, a bicistronic sa-mRNA vaccine encoding H5N1 hemagglutinin (HA) and neuraminidase (NA), in a Phase 1 randomized, observer-blind trial (NCT06602531). Healthy adults (N = 212) received two doses of ARCT-2304 (1.5, 5, or 12 mcg) or control/placebo on a 4- or 8-week schedule. Primary outcomes evaluated safety and reactogenicity (solicited/unsolicited/serious adverse events [AEs]) and immunogenicity (HA inhibition and NA enzyme-linked lectin assays 28 days post-second dose). Immunogenicity results were benchmarked using serum from a prior study supporting licensure of an H5N1 vaccine. ARCT-2304 was well-tolerated, displaying mostly mild-to-moderate, transient, dose-dependent solicited AEs, with no safety concerns identified. Two doses elicited robust HA and NA humoral responses. An 8-week interval yielded higher magnitude and greater durability than a 4-week schedule, with neutralizing and anti-NA antibodies remaining significantly elevated 6-7 months post-second dose. ARCT-2304 also induced dose-dependent, Th1-polarized T-cell responses peaking at Day 29. These findings support advancing sa-mRNA platforms for pandemic influenza preparedness.

Reoviridae-derived short trimerization domain for stabilizing homo-trimerization of vaccine immunogens and therapeutic proteins.

Wei Min M, Bu Qingfang Q, Xiao Jin J, Yuan Lunzhi L et al.

Trimerization motifs play pivotal roles in structural biology and therapeutic protein engineering. Here, we describe a short trimerization motif (rFd1303) derived from the Reoviridae family reovirus σ1 protein. Compared with the widely used T4-Foldon, the rFd1303 increased recombinant protein yields. The rFd1303-fused immunogens of SARS-CoV-2 spike and influenza hemagglutinin elicited antibody responses comparable to T4-Foldon-fused controls in mice. Using this tag, we engineered a trimeric ACE2-Ig fusion protein (TriACE2-Ig) that remained stable after 30 days at room temperature and neutralized a broad range of ACE2-utilizing sarbecoviruses, including 19 SARS-CoV-2 variants, SARS-CoV, and pangolin coronaviruses GD and GX, with higher potency than monomeric ACE2-Ig. In the hamster model challenged with various SARS-CoV-2 variants, our data demonstrated that intranasal TriACE2-Ig administration markedly reduced viral loads, virus-induced body-weight loss, lung pathology, and decreased within-cage virus transmission. These findings highlight rFd1303 as a versatile trimerization platform for vaccine and therapeutic protein development.

Divergent Human Papillomavirus Genotype Spectra in Three Indigenous Communities of Venezuela: Implications for Vaccine Choice and Cervical Screening.

Garcés María Fátima MF, Guzmán Silva Jefferson J, Aranguren Daniel D, Figueira Lina L et al.

Background: Genotype-resolved data on human papillomavirus (HPV) are almost absent for Venezuelan indigenous women. Methods: We studied 260 women from La Guajira (Wayuu, n = 94), Maniapure (Eñepa, n = 141), and Delta Amacuro (Warao, n = 25), processing all self-collected samples with a single real-time PCR assay that resolves 21 genotypes; cytology was available for 144. Results: HPV prevalence was 55.4% (95% CI 49.3-61.3) and high-risk prevalence was 51.2% (95% CI 45.1-57.2), rising from 42.6% in La Guajira to 59.6% in Maniapure and 80.0% in Delta Amacuro (p = 0.001). Nineteen genotypes were detected. HPV-31 was most frequent (15.8%), followed by HPV-16 (11.5%), and each community had its own profile, with HPV-68 and HPV-56 leading in Delta Amacuro. Half of the infected women carried more than one type. Cytology and HPV testing overlapped little: 20 of 78 HPV-positive women with a smear (25.6%) had an abnormal result (κ = 0.056). At least one nonavalent vaccine type was present in 69.4% of infected women, compared with 30.6% for the bivalent vaccine. Conclusions: HPV vaccination is not part of the Venezuelan national program; these data favor the nonavalent formulation, and HPV-based screening would need a feasible triage pathway for the many women who test positive.

Dietary L-Tryptophan Improves Growth, Antioxidant Status, Intestinal Barrier, and Resistance to Aeromonas hydrophila Infection in Largemouth Bass.

Zhang Xindang X, Wang Hengzhi H, Guo Chongchong C, Feng Shikun S et al.

Tryptophan (Trp) has been well documented to positively regulate nutrient metabolism, immune responses, and intestinal microecological homeostasis in mammals. However, relevant studies regarding its functions in largemouth bass remain limited. Herein, we assessed the impacts of dietary Trp on growth, antioxidant status, intestinal barrier, and disease resistance in largemouth bass. Fish were fed a basal control diet (C) or basal diets supplemented with 0.1%, 0.2%, 0.4%, and 0.8% L‑Trp for 60 days. Dietary L-Trp supplementation significantly improved FBW, WGR, and SGR of largemouth bass, with the maximum values found in the 0.4%Trp group. Based on the WGR and SGR data, quadratic regression analysis further identified 0.49% as the optimal dietary L-Trp inclusion level. In terms of antioxidant capacity, the 0.4%Trp group exhibited markedly higher serum and intestinal activities of CAT, GSH-PX, and SOD, as well as a higher content of T-AOC, compared to the C group. Moreover, the 0.4%Trp group significantly upregulated intestinal Nrf2 expression and downregulated Keap1 expression. Relative to the C group, dietary Trp supplementation significantly elevated intestinal villus height and muscular layer thickness in the 0.1%, 0.2%, and 0.4% Trp groups. Consistently, the mRNA levels of intestinal tight junction genes (ZO-1, Claudin-1, and Claudin-4) were upregulated in the 0.4%Trp group. Supplementation with 0.4%Trp significantly downregulated the expression of proinflammatory cytokines (TNF-α, IL-1β, and IL-8) and upregulated the expression of antiinflammatory cytokines (TGF-β1, IL-10, IL-22, and AHR). Intestinal microbial analysis further revealed that dietary L-Trp supplementation enriched the abundance of beneficial bacteria, reduced pathogenic microbial populations, and improved intestinal microbial diversity and richness. Finally, dietary L-Trp supplementation effectively improved the survival rate of largemouth bass following an Aeromonas hydrophila challenge. This was associated with enhanced serum activities of LZM, AKP, and ACP, as well as increased intestinal T-SOD activity. Additionally, L-Trp supplementation upregulated the expression of Nrf2, TGF-β1, IL-10, and IL-22. In contrast, the 0.4%Trp group reduced the MDA content and downregulated expression of Keap1, HSP70, TNF-α, IL-1β, and IL-8. Collectively, 0.4% dietary Trp strengthens antioxidant defense, remodels intestinal microbiota, improves intestinal physical and immune barriers, and enhances resistance against A. hydrophila infection in largemouth bass.
