Pharmaceutical Residues in Human Urine from Patients Under Antiretroviral Regimens and Self-Reported Healthy Individuals Before and After Three Months' Storage: Implication for Agricultural Reuse in Pretoria, South Africa.

Lizbelt Mugivhisa Liziwe ML, Faith Baloyi Khanyi BK, Oladeji Oluwaseun Mary OM, Olatunji Olatunde Stephen OS et al.

Background/Objectives: Pharmaceutical residues in human urine have raised serious concerns about the safe reuse of urine in agriculture. The current study investigates and compares the concentrations of pharmaceuticals in urine samples collected from male patients undergoing active antiretroviral regimens and self-reported "healthy individuals" at a government hospital in Pretoria, South Africa, and determines how the residues changed during three months of ambient storage, which is the usual recommendation before use in agriculture. Methods: The urine samples were collected in a plastic bottle to prevent fecal contamination, and 15 targeted compounds were analyzed using solid-phase extraction and liquid chromatography-mass spectrometry. The linearity ranged from 0.9910 to 0.9999, 75.5% to 99.1% for recovery, and intra-day and inter-day precision were below 6%. The urine from each group was collected into one pooled sample and then analyzed both pre- and post-storage. Results: Thirteen pharmaceuticals were detected in urine from patients on regular medication before storage; in "healthy individuals", detections rose from eight to twelve after storage. Concentrations in the composite samples after storage were higher for some of the compounds but lower for others, including caffeine, nevirapine, prednisolone and 17α-ethinylestradiol. Metformin was highest overall at 1186.25 µg/L and remained at 806.68 µg/L after storage. The highest concentration for all the pharmaceuticals was reported for metformin, with a value of 1186.25 µg/L, occurring in the group of patients on a regular regimen. There could be a possibility that the rise can be attributed to the hydrolysis of conjugated metabolites during storage; however, no analysis of conjugates was carried out. Conclusions: This study shows the need for appropriate treatment of human urine prior to its reuse as fertilizer and echoes the importance of further research into human exposure pathways and effective pharmaceutical removal strategies.

Terminal Oxygen as a Proton Acceptor: S-H Activation via Proton-Coupled Electron Transfer in Decavanadate-Catalyzed P(O)-S Coupling.

Wang Li Li LL, Shu Wei-Xuan WX, Wang Ya Y, Zhong Rong-Lin RL et al.

Polyoxometalates-catalyzed P-S oxidative coupling under mild conditions has emerged as a promising sustainable route to thiophosphates for pharmaceuticals and agrochemicals. This work reveals that the decavanadate cluster-catalyzed oxidative coupling between phosphine oxide and thiophenol involves four elementary steps, in which the rate-determining step is S-H bond activation via proton-coupled electron transfer (PCET). The polyoxovanadate cluster acts as a proton relay station, stabilizing the key proton in the transition state and facilitating its transfer. Interestingly, the proton preferentially transfers to a terminal oxygen rather than a bridging oxygen, with simultaneous electron transfer to the adjacent vanadium center. The terminal oxygen atom is more reactive than the bridging one because the [H3V10VO28]3- skeleton incurs a smaller electronic reorganization penalty for electron transfer in the terminal oxygen via a PCET pathway. On the other hand, the preferential activation of the S-H bond over the P-H bond in the [H3V10VO28]3--catalyzed reaction arises from both the smaller S-H bond dissociation energy and the better orbital energy match with the thiol substrate. These findings provide a theoretical foundation for understanding PCET in polyoxometalate catalysis and expanding the application of POMs in organic synthesis.

HDL-C as a mediator of PPCP-related kidney function decline in community-dwelling elderly: a mixture analysis approach.

Zhao Yutong Y, Wang Sheng S, Zhou Luoling L, Xu Yuanyuan Y et al.

Pharmaceuticals and personal care products (PPCPs) are widespread environmental contaminants, but their associations with kidney function and lipid metabolism remain incompletely understood. This study included 2758 adults aged ≥ 60 years in Anhui Province, China, during 2022-2023 and used a cross sectional design. Urinary concentrations of 48 PPCPs and two metabolites were measured using high-performance liquid chromatography-tandem mass spectrometry. Estimated glomerular filtration rate (eGFR) was calculated using the Chronic Kidney Disease Epidemiology Collaboration equation. Individual associations were assessed using logistic and linear regression, and mixture associations using weighted quantile sum (WQS) regression and Bayesian kernel machine regression. Restricted cubic splines examined nonlinear associations, and exploratory mediation analyses assessed the statistical contribution of high-density lipoprotein cholesterol (HDL-C). Among nine PPCPs with detection frequencies > 50%, doxycycline, enrofloxacin, acetaminophen, metformin, and cotinine were associated with lower eGFR after full adjustment and Benjamini-Hochberg correction. Coefficients ranged from - 3.00 to - 0.93 mL/min/1.73 m2 per tenfold increase in urinary concentration. Tylosin and caffeine were positively associated with eGFR. The WQS index was inversely associated with eGFR, with metformin receiving the largest weight. HDL-C showed a positive nonlinear association with eGFR. Under the mediation model assumptions, HDL-C statistically accounted for 12.21-29.30% of the associations involving enrofloxacin, metformin, and acetaminophen, and 13.78% of the mixture association. The indirect association for cotinine was nonsignificant. These findings remain exploratory. Prospective studies are needed to clarify temporal relationships and address possible reverse causation.

Development and validation of the analytical method for determination of residual solvents (2-propanol and n-hexane) in the drug substance Inosine Pranobex.

Chemna Khrystyna K, Logoyda Liliya L, Rudiuk Vitalii V

Monitoring residual solvents in active pharmaceutical ingredients (APIs) is critical due to their potential toxicity and impact on the physicochemical stability of the final medicinal product. Therefore, strict control of these impurities is a mandatory requirement to ensure patient safety and maintain compliance with international pharmacopoeia standards and good manufacturing practices (GMP). The goal of this study was to develop and validate a sensitive method for the determination of two residual solvents using headspace gas chromatography with flame ionization detector (HS-GC-FID) in Inosine Pranobex API according to The International Council for Harmonisation of Technical Requirements for Pharmaceuticals for Human Use (ICH) guidelines. A mixture of water and N, N-dimethylformamide (5:1) was selected as the sample solvent. To achieve high resolution between nonpolar n-hexane and polar 2-propanol, the method was implemented using a DB-624 (60 m × 0.25 mm × 1.4 μm) chromatographic column with an oven temperature program and at a flow rate of 35 cm/s. Equilibration time of the samples was 20 min at temperature 105 °С, total run time was 43 min. The analytical method complied with the established requirements during validation. The limit of quantification was confirmed for 2-propanol - 30 ppm, for n-hexane - 1 ppm. The developed method provides guaranteed purity and safety of the drug substance and is a reliable tool for controlling residual solvents at the drying stage of the technological process of synthesizing the drug substance Inosine Pranobex.

Contrasting Cannabinoid Receptor 2 (CB2R)-Associated Responses in Two Different Cerebrovascular Endothelial Cell Lines in a Blood Brain Barrier Model in the Context of HIV.

Delorme-Walker Violaine V, Au Kaylin K, Lim Wei Ling WL, Sasaki Takayo T et al.

Despite the success of suppressive antiretrovirals, infection with human immunodeficiency virus (HIV) is associated with several comorbidities which include negative consequences to the central nervous system (CNS), where disruption of the blood-brain barrier (BBB) is a major underlying factor in the resulting chronic inflammation and contribution to the pathogenesis of neuroHIV. Currently, the use of cannabis and cannabinoid derivatives among persons living with HIV (PWH) is common. Despite perceived benefits, we have previously identified context-dependent effects of cannabis use, including its impact on vascular biomarkers. In this study, we used an in vitro polycellular BBB model with two different human cerebrovascular endothelial cell lines (hCMEC/D3 and HBMEC/ci18) to test the effects of cannabinoids and their receptors on barrier integrity and function in the context of the latent HIV brain environment, mimicked by the addition of conditioned media from HIV latently infected promonocytes. We found that the two brain endothelial cell lines had similar responses to HIV-conditioned media by increasing permeability to dextran and decreased expression of tight junction proteins. However, their response to cannabinoids, particularly via the cannabinoid receptor 2 (CB2R), was markedly different, with hCMEC/D3 cells showing improvement of BBB integrity by all measures, but HBMEC/ci18 cells showing no benefits or aggravation of damage. While the contrasting effects were not due to differences in cell viability or proliferation, differences CB2R-elicited baseline G protein-coupled receptor (GPCR) response thresholds between the two cell lines were significant, revealed by forskolin-induced cAMP levels that were 50-200-fold higher in hCMEC/D3 cells in comparison to HBMEC/ci18. This was correlated to higher baseline CB2R expression in hCMEC/D3, and was not an HIV-induced signaling defect in HBMEC/ci18. The CBD/THC-induced phosphorylation of p38 MAPK was also significantly stronger in hCMEC/D3 compared to HBMEC/ci18, was abrogated by a CB2R inverse agonist/antagonist. Motivated by the need to identify predictors of positive response to cannabinoids, our study suggests that CB2R levels and GPCR activation thresholds in cerebrovascular endothelium may be important candidate mechanistic biomarkers to be tested in primary cells, animal models, and longitudinal clinical cohorts, with the goal of distinguishing beneficial versus aggravating effects of cannabis to the BBB, particularly in the context of HIV.

Biochemical Characterization and Stability of a New Fibrinogen Concentrate (BT524) Manufactured Using an Innovative Virus Inactivation Step.

Schulte Linda L, Fürtig Boris B, Scheich Christoph C, Kistner Steffen S et al.

Human plasma is utilized in the production of life-saving pharmaceuticals, including fibrinogen, a key player in clot formation, tissue repair, and hemostasis. Fibrinogen deficiency results in impaired blood clotting and potentially severe hemorrhagic events. Fibrinogen concentrate or cryoprecipitate are the preferred options for fibrinogen replacement therapy. Both increase plasma fibrinogen levels, but utilization of fibrinogen concentrate is advantageous. It offers enhanced pathogen safety through manufacturing procedures that remove or inactivate viruses or prions potentially present in human plasma. We developed a novel manufacturing process that yields a fibrinogen concentrate (BT524) and coagulation factor VIII (Haemoctin® SDH) in one workflow. This process contains several virus inactivation steps, including an innovative UV-C irradiation and a cation exchange chromatography (CEX) step for prion removal. While successfully inactivating potential viral contamination, UV-C irradiation showed no influence on fibrinogen structure and composition, and no formation of neoantigenic structures could be identified. Fibrinogen concentrate (BT524) contained almost exclusively fibrinogen with activity close to 100%, very low numbers of sub-visible particles and lacked measurable thrombogenic activity. The stability study of clinical material demonstrated a stable product for five years under several storage conditions. With a solubility at room temperature of 2-4 min upon reconstitution using the nextaro®v transfer device, the fibrinogen concentrate is almost immediately ready to use. The reconstituted solution demonstrated physico-chemical stability for 24 h. Fibrinogen concentrate (BT524) generated using the novel manufacturing process yielded a product of high quality, purity and virus safety. In addition, it offers excellent solubility, long-term stability and high activity. The integration of shared process steps with Haemoctin® SDH facilitates a more ethical and efficient utilization of human blood plasma. In clinical trials, the potency, safety and tolerability of the manufactured fibrinogen concentrate were demonstrated.
