Theranekron Attenuates Inflammation, Oxidative Stress Index, and Apoptosis in Lipopolysaccharide-Induced Cardiovascular Toxicity: Association With Hypoxia-Inducible Factor-1 Alpha-Related Gene Expression.

Savran Mehtap M, Asci Halil H, Imeci Orhan O, Oguzlar Furkan Cagri FC et al.

This study aimed to evaluate the possible protective effects of Theranekron, an extract derived from Tarantula cubensis venom, on cardiovascular damage induced by lipopolysaccharide (LPS) in an experimental rat model. A total of 32 male Wistar Albino rats were randomly divided into 4 groups: Control, Lipopolysaccharide, Lipopolysaccharide plus Theranekron, and Theranekron alone. Lipopolysaccharide administration caused pronounced histopathological changes in the heart and aorta, including endothelial sloughing, hyperemia, hemorrhage, and infiltration of inflammatory cells. These pathological findings were accompanied by elevated oxidative stress markers, increased tumor necrosis factor-alpha and inducible nitric oxide synthase immunoreactivity, and increased caspase-3 immunoreactivity. Additionally, genes associated with the hypoxia-inducible factor-1 alpha (HIF-1α) pathway-such as interleukin 6, signal transducer and activator of transcription 3 (STAT3), and vascular endothelial growth factor-were significantly upregulated in the lipopolysaccharide group. Theranekron pretreatment was associated with partial attenuation of oxidative stress imbalance, of inflammatory and apoptotic immunoreactivity, and of the upregulation of HIF-1α-related transcripts. Overall, these findings suggest that Theranekron may offer partial protection against early LPS-induced cardiovascular injury.

Therapeutic Potential of Bone Marrow-Derived Mesenchymal Stem Cells in A Letrozole-Induced Rat Model of Polycystic Ovary Syndrome.

M Abou-El-Naga Amoura A, Younes Elarby Nada N, El-Dusoky Sara S, Fayad Eman E et al.

Polycystic ovary syndrome (PCOS) is a prevalent metabolic and reproductive disorder and one of the leading causes of infertility. Bone marrow-derived mesenchymal stem cells (BM-MSCs) possess anti-inflammatory properties and have shown potential in alleviating PCOS manifestations in experimental models. This study aimed to evaluate the therapeutic efficacy of transplanted BM-MSCs and BM-MSC-derived culture media in a letrozole (LTZ)- induced PCOS rat model. The experimental study was conducted on 50 female Sprague Dawley rats aged (4-6 weeks) and weighing (100-120 g), using a LTZ-induced PCOS model. Animals were randomly assigned to 4 groups, including control, PCOS, BM-MSC-treated, and BM-MSC culture media-treated groups. BM-MSCs were isolated from (bone marrow) and administered via (tail vain) at a dose of (1×106 BM-MSC). Blood samples were collected to measure hormonal and metabolic parameters (insulin, androgen, progesterone, estrogen, follicle-stimulating hormone (FSH), and luteinizing hormone (LH) using [enzyme-linked immunosorbent assay (ELISA) technique). Ovarian tissues were analyzed by for histopathological and immunohistochemical methods to assess [key markers, e.g., follicular development, tumor necrosis factor-alpha (TNF), and vascular endothelial growth factor (VEGF)]. The PCOS group exhibited hormonal imbalance, metabolic disturbance, and ovarian dysfunction. BMMSC treatment significantly reduced insulin (P=0.003) and androgen levels (P=0.040), while restoring progesterone (P=0.003), estrogen (P=0.007), FSH (P=0.009), and LH (P=0.001) to near-normal levels. BM-MSCs markedly improved both metabolic and reproductive parameters, effectively restoring fertility in the PCOS model. Treatment with BM-MSC-derived culture media produced partial improvement but did not fully reverse PCOS manifestations. BM-MSCs improved ovarian structure and function, ameliorating both metabolic and reproductive dysfunction in the PCOS rat model. BM-MSC transplantation yielded the most favorable outcomes, whereas BM-MSCderived culture media alone provided incomplete therapeutic benefits.

The potential of ruxolitinib as a senomorphic therapy in alleviating neovascular age-related macular degeneration : Ruxolitinib inhibits CNV.

Wang Yinhao Y, Huang Zhihua Z, Tseng Yulin Y, Ma Huiling H et al.

Cellular senescence plays a key role in the pathogenesis of neovascular age-related macular degeneration (nAMD). Senomorphic therapy, which targets the senescence-associated secretory phenotype (SASP), holds promise for treating age-related diseases; however, effective senomorphic treatments for nAMD remain lacking. We screened a panel of senomorphic agents in bleomycin- and ionizing radiation (IR)-induced senescent ARPE-19 cells to identify a lead compound for nAMD. Bulk and single-cell RNA sequencing data were analyzed to explore JAK-STAT signaling activation. Ruxolitinib and siRNA were used to validate the role of the JAK1/2-STAT3 pathway in senescent retinal pigment epithelium (RPE). A laser-induced choroidal neovascularization (CNV) model was established in Brown-Norway rats for in vivo studies. Ruxolitinib, a JAK1/2 inhibitor, most effectively suppressed SASP factors and complement C3 expression while restoring pigment epithelium-derived factor (PEDF) levels in senescent ARPE-19 cells. Knockdown of JAK1 or JAK2 significantly reversed the expression of vascular endothelial growth factor (VEGF), C3, and PEDF. Gene set enrichment analysis (GSEA) and gene set variation analysis (GSVA) revealed activation of the JAK-STAT pathway in nAMD patient samples, CNV models, and senescent RPE cells, which was corroborated in the CNV rat model. Inhibition of JAK1/2-STAT3 with ruxolitinib markedly reduced vascular leakage and CNV area, and accelerated recovery of retinal function. The combination of ruxolitinib with anti-VEGF therapy presented a better effect on inhibiting CNV area. Furthermore, intravitreal injection of ruxolitinib showed no significant structural or functional toxicity in normal rat retinas over one month. Our study identifies the JAK1/2-STAT3 pathway as a key regulator of the SASP in senescent RPE and confirms its activation in nAMD and experimental CNV. Pharmacologic inhibition of this pathway with ruxolitinib effectively attenuates CNV progression with a favorable short-term safety profile, supporting its potential as a senomorphic therapy for nAMD.

Beyond pain: early Hypotonia and growth failure in congenital insensitivity to pain with Anhidrosis.

Md Sabudin Siti Nur Syakinah SNS, Draman Nani N, Ramali Khairul Nazli KN

Congenital insensitivity to pain with anhidrosis is a rare genetic disorder caused by mutations in the neurotrophic receptor tyrosine kinase 1 gene, characterized by absent pain perception and inability to sweat. Early recognition is essential to prevent severe complications. We report a seven-month-old boy presenting with poor growth, hypotonia, and delayed motor development. Initial metabolic and neuromuscular evaluations, including nerve conduction studies and brain imaging, were normal. His growth remained at the minus three standard deviation centile. By twelve months, additional features emerged, including self-injurious behavior, lack of response to injury, and reduced sweating. A positive family history prompted genetic testing, which confirmed a pathogenic variant in the neurotrophic receptor tyrosine kinase 1 gene. This case highlights that early nonspecific features may precede classical manifestations, emphasizing the importance of clinical vigilance, family history, and early genetic testing to enable timely multidisciplinary management.

The peripheral neuromuscular continuum: Histological remodeling induced by physical training.

Neto Walter Krause WK, Gama Eliane Florencio EF

Physical exercise induces structural adaptations that extend far beyond skeletal muscle, affecting virtually every component of the peripheral neuromuscular system. Although remodeling of peripheral nerves and the neuromuscular junction (NMJ) has been increasingly documented, current knowledge remains fragmented because researchers have largely studied these structures in isolation rather than as components of a continuous biological system. This review critically synthesizes evidence on histological, ultrastructural, and cellular adaptations induced by physical exercise throughout the peripheral neuromuscular system. It emphasizes remodeling of peripheral nerve fibers, Schwann cells, the blood-nerve barrier, the extracellular matrix, and the NMJ, as well as the cellular and molecular mechanisms that coordinate these responses. The review also examines how aging affects peripheral nerve and NMJ integrity, discusses current methodological limitations, and proposes an integrative framework that interprets exercise-induced adaptation as coordinated remodeling across the peripheral neuromuscular continuum. Collectively, the available evidence indicates that regular physical exercise preserves structural integrity throughout the neuromuscular system, providing a broader biological basis for its protective effects than skeletal muscle adaptations alone.

Comparison of Vocal Cord Function in Patients Undergoing Thyroid Surgery With and Without Microscopic Assistance.

Kaur Upinder U, Singh Gurshinderpal G, Prinja Sumit S

Introduction Recurrent laryngeal nerve injury is a complication of thyroid surgery that can affect vocal cord function. Microscopic magnification may improve the visualization of critical anatomical structures and facilitate precise dissections. This study aimed to compare the vocal cord function and postoperative outcomes of patients who underwent thyroid surgery with and without microscopic assistance. Materials and methods This hospital-based prospective comparative observational study included patients undergoing hemithyroidectomy or total thyroidectomy at the Department of Otorhinolaryngology. Preoperative and postoperative vocal cord function was assessed using laryngoscopy. Surgical and postoperative outcomes were compared between the groups undergoing microscope-assisted and conventional thyroidectomy. Statistical analysis was performed with p < 0.05 considered statistically significant. Results A total of 166 patients were included, comprising 76 (45.8%) and 90 (54.2%) patients in the microscope and conventional groups, respectively. Hemithyroidectomy was performed in 106 (63.9%) patients and was more frequent in microscopy (73.7% vs. 55.6%; p = 0.016). Operative time was significantly longer with microscopy for hemithyroidectomy (114.35 ± 6.7 vs. 96.0 ± 7.59 min; p < 0.001) and total thyroidectomy (130.38 ± 2.7 vs. 110.5 ± 2.64 min; p < 0.001). Transient recurrent laryngeal nerve palsy occurred in three (3.9%) versus 11 (12.2%) patients (p = 0.056), whereas hematoma was significantly lower with microscopy (1.3% vs. 8.9%; p = 0.032). In hemithyroidectomy, any recurrent laryngeal nerve palsy was significantly lower with microscopy (3.6% vs. 16.0%; OR = 0.19, 95% CI: 0.04-0.96; p = 0.046). Conclusion Microscopic thyroidectomy may improve recurrent laryngeal nerve preservation, particularly during hemithyroidectomy, although it requires a longer operative time.
