Treatment Approach to Disseminated Peritoneal Leiomyomatosis in A Patient Desiring Future Fertility: A Case Report.

Neal Samantha E SE, Ryles Hannah T HT, Koomson Christabel C, Madueke-Laveaux Obianuju Sandra OS et al.

Disseminated peritoneal leiomyomatosis (DPL) is a rare condition in which fibroids diffusely grow in extrauterine locations. Given its low prevalence, the literature is limited, making evidence-based treatment particularly challenging, especially in premenopausal patients. This case report describes a 34-year-old nulligravid woman with a history of two prior myomectomies who presented with upper abdominal bulk symptoms. Abdominal and pelvic magnetic resonance imaging (MRI) revealed multiple mesenteric and omental masses concerning for recurrent DPL. She underwent exploratory laparotomy with removal of 13 abdominal fibroids ranging from 2 to 12 cm. Postoperatively, her symptoms improved, and she was treated with a gonadotropin-releasing hormone (GnRH) analog (relugolix/estradiol/ norethindrone acetate) with a favorable response and sustained remission on follow up. Our case report demonstrates the efficacy of a novel medical therapy and our scoping review highlights the importance of aggressive surgical management followed by medical therapy for the treatment of DPL in patients with future fertility desires.

Low-dose CT denoising using a diffusion prior via score distillation sampling.

Jung Hojin H, Lee Jooho J, Baek Jongduk J

Low-dose computed tomography (LDCT) reduces radiation exposure but amplifies noise, which can degrade diagnostic reliability. Recent studies have applied diffusion-based models to LDCT denoising and shown promising performance. However, many of these approaches require paired LDCT and normal-dose CT (NDCT) data or dose-specific retraining, and their reconstruction procedures are often strictly coupled to a predefined diffusion trajectory. This study proposes an optimization-based LDCT denoising framework that uses a pre-trained NDCT diffusion model as an image prior and can be applied across various dose levels without dose-specific retraining. We formulate LDCT denoising as an optimization problem guided by a pre-trained score-based generative model (SGM) trained only on NDCT images. The Poisson data fidelity term enforces consistency with the measured projection data, whereas the score distillation sampling (SDS) term acts as a regularizer that guides the reconstruction toward the NDCT distribution learned by the pre-trained SGM. The denoised image is obtained by iteratively optimizing a projection-domain residual variable using the combined Poisson data fidelity gradient and SDS-based regularization gradient. The proposed method effectively suppressed noise while preserving fine anatomical structures across multiple dose levels using a single pre-trained diffusion model. Quantitative evaluations with respect to full-dose reference images and visual comparisons demonstrated competitive performance compared with supervised and self-supervised LDCT denoising baselines. The results show that diffusion-based image priors can be leveraged across various low-dose levels within the proposed optimization-based denoising framework. Our work suggests that SDS-guided diffusion priors are a promising direction for LDCT denoising in data-limited settings.

Short-Term Adverse Events Following Recombinant Zoster Vaccination in Older Adults in Australia: A Survey Study Comparing Dose 1 and Dose 2 and Influence of Dose Interval.

Dawes Lucy L, Nguyen Thuy T, Macartney Kristine K, Deng Lucy L

Self-reported adverse events following first and second recombinant zoster vaccination (RZV) doses in older adults (n = 41,069) collected through AusVaxSafety were analysed. Adverse event rates were higher following Dose 2 (54%; 95% CI, 53%-54%) than after Dose 1 (45%; 95% CI, 45%-46%), particularly with 3-6- and 7-9-month dose intervals, although medical review rates remained low (1.1%; 95% CI, 1.0%-1.2%), suggesting RZV is reactogenic but with no serious outcomes.

Selective Estradiol Sensitivity in 12Z Human Endometriotic Epithelial Cell Line.

Banerjee Shohini S, Chopra Esha E, Smith Ian M IM, Stroka Kimberly M KM

Estradiol (E2) is a potent estrogen molecule that plays a crucial role in regulating numerous healthy and pathophysiological processes. To model estrogen-dependent cellular activities in vitro in a physiologically meaningful way, cells chosen for experimentation should exhibit sensitivity to E2. The 12Z human endometriotic epithelial cell line is gaining attention as a model for estrogen-dependent conditions, but its functional responsiveness to E2 has not been well characterized. For the passage range in the following study, we found that E2 does not statistically significantly influence 12Z proliferation, wound closure, or single-cell migration. However, 12Zs were selectively responsive to E2 in 3D migration models where the cells were physically confined. Upregulating ESR1, the primary mediator of estrogenic action, in the 12Zs did not enhance their functional E2 sensitivity. These results highlight the importance of verifying hormone responsiveness when using 12Z cells to model estrogen-dependent processes and conditions.

Hematological Parameters and Occupational Ionizing Radiation Exposure in Interventional Healthcare Workers: A Longitudinal Matched Case-Control Study.

d'Ettorre Gabriele G, Loiacono Giuseppe G, Giannelli Gianmarco G, Piscitelli Prisco P et al.

The rising volume of fluoroscopy-guided interventions poses an occupational safety challenge for healthcare workers (HCWs) exposed to chronic low-dose ionizing radiation (LDR), which carries stochastic risks of bone marrow suppression and radiation-induced cytopenias. Real-world longitudinal evidence validating the efficacy of modern protection standards on specific peripheral hematological parameters remains limited. This study aims to evaluate the peripheral hemogram profiles of interventional HCWs exposed to LDR, compared to those of a non-exposed control cohort, from baseline (T0) to follow-up (T1). A longitudinal matched case-control study evaluated the complete blood count with differential leukograms of 136 interventional HCWs exposed to LDR compared to those of 272 matched non-exposed controls from baseline (T0) to follow-up (T1), stratifying by role, seniority, and cumulative ionizing radiation dose. The mean annual radiation dose was 0.22 mSv, and the mean cumulative radiation dose was 4.11 mSv. Hemogram indicators showed no significant differences between cases and controls at T0 and T1 (p > 0.05). Stratification by length of service (<10 vs. >10 years), and radiation dose (<3 vs. ≥3 mSv) demonstrated no significant variations in erythrocyte, leukocyte, or platelet counts. Regression analysis confirmed no association between hematological parameters and job seniority or radiation dose. All radiation doses remained below mandatory limits. The findings provide objective evidence of the effectiveness of current safety measures countering radiation-induced hematotoxicity in hospital settings. Longitudinal tracking confirms the absence of adverse hematological shifts, suggesting that structured training and strict personal protective equipment (PPE) use mitigate hematotoxicity risks among interventional staff exposed to ionizing radiation in healthcare settings. Safety protocols should maintain strict PPE compliance and safety training. Occupational health policy needs to prioritize continuous dosimetry monitoring, as routine hematological screening may not show early low-dose exposure effects.

Joint Association of Longitudinal Single-Point Insulin Sensitivity Estimator Patterns and Hypertension Status with Incident Stroke Among Adults with Cardiovascular-Kidney-Metabolic Syndrome Stages 0-3: A Nationwide Prospective Cohort Study.

Sun Jibo J, Ma Xiao X, Zhang Xue X, Liu Guanping G et al.

The prognostic value of longitudinal single-point insulin sensitivity estimator (SPISE) patterns for stroke, particularly when considered in combination with hypertension (HTN), remains unclear among adults with cardiovascular-kidney-metabolic (CKM) syndrome stages 0-3. This study aimed to examine the association of combined longitudinal SPISE patterns and HTN status with incident stroke among adults with CKM syndrome stages 0-3. A nationwide prospective cohort study. We included adults aged ≥ 45 years with CKM stages 0-3 from the China Health and Retirement Longitudinal Study. SPISE was calculated at Waves 1 and 3, and incident stroke was ascertained through Wave 5. K-means clustering was used to identify longitudinal SPISE patterns, which were dichotomized as persistently low or non-persistently low. Participants were then cross-classified according to longitudinal SPISE pattern and baseline HTN status. Cumulative SPISE exposure was calculated from repeated SPISE measurements. Cox models were used to examine associations with incident stroke. Dose-response, subgroup, sensitivity, and model-performance analyses were also performed. Among 4,443 participants, 348 incident strokes occurred. A persistently low longitudinal SPISE pattern was associated with a higher risk of incident stroke than a persistently high pattern [hazard ratio (HR), 2.29; 95% confidence interval (CI), 1.59-3.29]. Compared with the group with no HTN and a non-persistently low SPISE pattern, stroke risk was higher in the group with no HTN and a persistently low SPISE pattern (HR, 1.48; 95% CI, 1.12-1.95), the group with HTN and a non-persistently low SPISE pattern (HR, 1.57; 95% CI, 1.10-2.23), and the group with HTN and a persistently low SPISE pattern (HR, 2.45; 95% CI, 1.84-3.26). Higher cumulative SPISE exposure was inversely associated with incident stroke (HR per 1-unit increment, 0.95; 95% CI, 0.93-0.97), with an approximately linear dose-response relationship. Incorporating the combined longitudinal SPISE pattern and HTN status increased the apparent C-index from 0.612 to 0.646. However, optimism-corrected discrimination was comparable between the combined model and the HTN-only model. The combination of longitudinal SPISE patterns and HTN status identified a subgroup at the highest risk of incident stroke in this CKM population.
