Validation of the Brief Pathological Narcissism Inventory and Its Association with Cyber Intimate Partner Violence in Portuguese Emerging Adults.

Rego Mónica M, Caridade Sónia S

Cyber intimate partner violence (C-IPV) refers to abusive and controlling behaviors enacted through digital technologies toward a current or former intimate partner. Previous research suggests that pathological narcissism may be associated with C-IPV. This study examined the psychometric properties of the Brief Pathological Narcissism Inventory (B-PNI), the frequency of self-reported C-IPV perpetration and related behaviors, and the association between C-IPV perpetration and pathological narcissism among emerging adults. The sample comprised 371 participants aged 18-25 years (M = 21.52, SD = 1.97), of whom 78.7% were female. Confirmatory factor analysis provided support for the a priori two-factor model of the B-PNI (CFI = 0.982, TLI = 0.965, RMSEA = 0.076), comprising grandiose and vulnerable narcissism. The B-PNI showed satisfactory internal consistency and theoretically expected associations with psychopathological symptoms and broader personality dimensions. Overall, 79.5% of participants reported at least one C-IPV behavior, with 61.5% reporting online control, 62.0% online jealousy, and 31.5% online intrusive behaviors. C-IPV perpetration was positively and significantly associated with both grandiose and vulnerable narcissism, although the associations were small in magnitude. The findings provide preliminary support for the use of the B-PNI among Portuguese emerging adults and indicate that pathological narcissism is associated with C-IPV perpetration. Given the modest magnitude of these associations, narcissistic traits should be considered as one of multiple individual and relational factors potentially involved in C-IPV.

Clinical Pathway Management for Platelet-Rich Plasma Therapy in Kellgren-Lawrence Grade I and II Knee Osteoarthritis: A Prospective, Multicenter, Double-Blind Randomized Controlled Trial.

Yang Shunjie S, Wang Hui H, Liu Yan Y, Fu Fangang F et al.

Clinical outcomes after platelet-rich plasma (PRP) treatment for knee osteoarthritis are inconsistent, and some well-designed trials show no benefit. Whether a standardized multicomponent pathway improves outcomes relative to conventional PRP care is uncertain. To compare the efficacy, safety, satisfaction, and costs of a standardized PRP pathway versus physician-directed conventional PRP care in Kellgren-Lawrence grade I and II knee osteoarthritis. Randomized controlled trial; Level of evidence, 1. At 3 hospitals, 240 patients were randomized 1:1 to a standardized PRP clinical pathway (PRP-CP; n = 120) or physician-directed conventional PRP care (PRP-C; n = 120). PRP-CP combined standardized selection, quality-controlled leukocyte-poor PRP, ultrasound-guided injections, staged rehabilitation, and systematic follow-up. Primary 12-month outcomes were visual analog scale (VAS) pain, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) total, and International Knee Documentation Committee (IKDC) scores; secondary outcomes included achievement of the minimal clinically important difference, satisfaction, adverse events, and costs. At 12 months, PRP-CP produced greater changes than PRP-C in VAS (mean difference [MD], -2.98 [95% CI, -3.64 to -2.32]), WOMAC total (MD, -8.63 [95% CI, -11.35 to -5.90]), and IKDC (MD, 10.25 [95% CI, 8.37 to 12.13]) scores (all P < .001). Achievement of the minimal clinically important difference favored PRP-CP at 3, 6, and 12 months for the VAS and at 6 and 12 months for the IKDC and WOMAC (P < .05). Satisfaction was greater with PRP-CP (odds ratio, 1.79 [95% CI, 1.11-2.89]; P = .017). An adverse event occurred in 17 of 115 patients in the PRP-CP group (14.8%) versus 36 of 113 patients in the PRP-C group (31.9%) (Fisher exact test: P = .003). Mean costs were CN¥15,199.9 versus CN¥14,754.4, respectively; the incremental cost-effectiveness ratio was CN¥66.0 per WOMAC point. Effects were generally consistent across centers, and blinding was maintained. The standardized multicomponent pathway produced greater 12-month improvements in pain and function than conventional PRP care, with fewer adverse events. Because components were bundled and no placebo group was included, the trial does not establish PRP efficacy or component-specific effects.

Increasing Language Access for Intimate Partner Violence Support Services: A Scoping Review.

Richey Ann E AE, DiMascolo Lisa D LD, Bender Anna E AE, Bhattacharya Anindita A et al.

Intimate partner violence (IPV) is a significant public health concern. Survivors face numerous barriers to accessing services, particularly when services in many places are only provided in English. This scoping review aimed to identify research and practice-based articles that explore interventions to increase language accessibility to support services for IPV. Articles were identified through a systematic search of databases and selected for inclusion if they involved language accessibility for IPV support services across settings (e.g., legal services, advocacy, hotlines, social services, healthcare, law enforcement). We identified 15 research and 14 practice-based articles for inclusion. Most interventions and practices for increasing language access were in settings related to health care, advocacy and social services, or community-based organizations. Research articles described various adaptations to interventions for increasing language access, which mostly included interpretation, translation, bilingual assistance, and culturally tailored programs. All research articles had language access adaptations for their intervention, but only 60% of the research articles included specific findings related to language access and culture. Practice-based articles discussed guidance around the development, assessment, and evaluation of a language access plan, language access engagement and implementation, and different elements and settings for language access. Findings highlight the intersection between language and culture, underscoring the need to address these interconnected aspects beyond simple translation and interpretation. IPV support services should hire multilingual and multicultural staff and address fear around immigration status. Increased funding is needed to ensure higher quality language access supports are implemented. Additional implications for research, practice, and policy are discussed.

The Value of the White Blood Cell-to-Hemoglobin Ratio in Acute Lower Respiratory Tract Infections of Pseudomonas Aeruginosa.

Hong Mei M, Zhang Da-Wei DW

The white blood cell to hemoglobin ratio (WBC/Hb) shows potential in predicting the severity and poor prognosis of infectious diseases. However, the specific utility of WBC/Hb in acute lower respiratory tract infections caused by Pseudomonas aeruginosa (ALRTIs-PA) has not been fully elucidated. This study aimed to evaluate the diagnostic value of WBC/Hb for disease severity (ICU admission) and its prognostic value for 90-day mortality in patients with ALRTIs-PA. A retrospective study was conducted among patients with ALRTIs-PA admitted to the First Affiliated Hospital of Anhui Medical University from January 2020 to January 2024. Baseline demographics, clinical characteristics, and laboratory parameters were extracted from electronic medical records. Logistic regression analysis was used to identify independent risk factors for ICU admission. A Cox proportional hazards model was used to determine predictors of 90-day all-cause mortality, and significant variables were subsequently into a prognostic prediction model. A total of 549 patients were enrolled, of whom 301 (54.8%) ICU admission, and 72 (13.1%) died within 90 days. Multivariate logistic regression analysis identified WBC/Hb, total protein (TP), blood urea nitrogen (BUN), lactate dehydrogenase (LDH), and neutrophil to lymphocyte ratio (NLR) as independent risk factors for ICU admission. Receiver operating characteristic (ROC) curve showed the best predictive value of WBC/Hb. Multivariable Cox regression analysis identified WBC/Hb, along with NLR, TP, tracheotomy, tracheal intubation, mechanical ventilation, acute renal failure, and the number of antibiotics used, as independent risk factors for 90-day mortality in patients with ALRTIs-PA. Notably, WBC/Hb was the strongest predictor (HR =8.130, 95% CI: 3.852-17.157, P < 0.001) of mortality among all examined variables. Our findings suggest that WBC/Hb represents a valuable predictor for ICU admission and 90-day mortality in patients with ALRTIs-PA.

Extended dosing intervals enhance the durability of a self-amplifying mRNA vaccine against pandemic H5N1 influenza: a randomized, observer-blind, dose-finding Phase I study.

Pragalos Antoinette A AA, Pacciarini Filippo F, Jin Hongfan H, Hopkins Terri T et al.

Avian influenza poses a persistent pandemic threat, yet traditional vaccines face production delays. Self-amplifying mRNA (sa-mRNA) technology offers potential advantages, including rapid development, dose sparing, and enhanced, durable immunity. We evaluated ARCT-2304, a bicistronic sa-mRNA vaccine encoding H5N1 hemagglutinin (HA) and neuraminidase (NA), in a Phase 1 randomized, observer-blind trial (NCT06602531). Healthy adults (N = 212) received two doses of ARCT-2304 (1.5, 5, or 12 mcg) or control/placebo on a 4- or 8-week schedule. Primary outcomes evaluated safety and reactogenicity (solicited/unsolicited/serious adverse events [AEs]) and immunogenicity (HA inhibition and NA enzyme-linked lectin assays 28 days post-second dose). Immunogenicity results were benchmarked using serum from a prior study supporting licensure of an H5N1 vaccine. ARCT-2304 was well-tolerated, displaying mostly mild-to-moderate, transient, dose-dependent solicited AEs, with no safety concerns identified. Two doses elicited robust HA and NA humoral responses. An 8-week interval yielded higher magnitude and greater durability than a 4-week schedule, with neutralizing and anti-NA antibodies remaining significantly elevated 6-7 months post-second dose. ARCT-2304 also induced dose-dependent, Th1-polarized T-cell responses peaking at Day 29. These findings support advancing sa-mRNA platforms for pandemic influenza preparedness.
