Knowledge of Hypertensive Disorders of Pregnancy and Associated Factors Among Antenatal-Care Attendees in Mbarara City, Southwestern Uganda: A Cross-Sectional Study.

Lugobe Henry Mark HM, Migisha Richard R, Wilkinson Ian B IB, Bajunirwe Francis F et al.

Hypertensive disorders of pregnancy (HDP) are important contributors to maternal and perinatal morbidity and mortality in sub-Saharan Africa. Women's knowledge of warning signs, risk factors, diagnosis, and prevention may support earlier care-seeking and timely diagnosis, yet data from primary-level antenatal-care settings in Uganda remain limited. We assessed knowledge of HDP and associated factors among antenatal-care attendees in Mbarara City, southwestern Uganda. We conducted a cross-sectional study among pregnant women attending their third or later antenatal-care visit at six public health facilities in Mbarara City between January and March 2025. Participants were recruited by consecutive sampling. Knowledge was assessed using 41 scored knowledge items covering awareness, disease manifestations, risk factors, diagnosis, management, prevention, complications, and blood-pressure monitoring. Scores were expressed as percentages of correct responses; adequate knowledge was defined as ≥60%. Multivariable modified Poisson regression was used to estimate adjusted prevalence ratios. Among 426 antenatal-care attendees, 271 had adequate knowledge, giving a prevalence of 63.6% (95% CI: 58.8-68.2). Knowledge was highest for timing or frequency of blood-pressure checks (86.2%) and complications (76.1%), but lower for diagnosis (51.4%), timing of occurrence (53.3%), treatment and management (53.4%), and risk factors (56.8%). Adequate knowledge was more prevalent among women with secondary education (aPR=1.34; 95% CI: 1.10-1.64) or tertiary education (aPR=1.36; 95% CI: 1.04-1.78), compared with primary education or less; women with five or more pregnancies compared with primigravidas (aPR=1.77; 95% CI: 1.27-2.48); and women with family history of preeclampsia compared with those without (aPR=1.26; 95% CI: 1.06-1.50). Approximately two-thirds of antenatal-care attendees had adequate knowledge of HDP, but important gaps remained in diagnosis, timing of occurrence, treatment and management, and risk-factor recognition. Health education during antenatal care should prioritize practical messages for women with lower educational attainment, primigravidas or limited pregnancy experience, and women without a family history of preeclampsia.

Fueling Bright Futures: a mixed-methods case study on integrating gardens, canteens, and nutrition training in philippine public schools.

Hoang Mai A MA, Matienzo Patricia Isabel A PIA, Umali Diana Jean DJ, Anunciado Ma Shiela MS et al.

This mixed-methods case study evaluates Fueling Bright Futures, an integrated approach involving enhancing school gardens, improving the menu of canteen, and providing nutrition education in three public schools in Cavite, Philippines. The intervention addresses both undernutrition and overweight and obesity, tackling the double burden of malnutrition among Filipino schoolchildren. This mixed-methods study was conducted in three public elementary schools in Cavite, Philippines. Baseline and endline assessments (Apr 2024, Feb 2025) included canteen menu audits (per DepEd Order 13), garden inventories, and focus groups with staff and administrators. Effectiveness and scalability were evaluated using the Intervention Scalability Assessment Tool (ISAT) in participatory workshops with policymakers and stakeholders. Qualitative data from all sessions were coded and thematically analyzed via NVivo14 and mapped to ISAT domains. All the schools made substantial gains in garden output, dietary diversity, and the nutritional quality of canteen offerings. The number of "green" menu items increased sharply (School A: 2-11 per week), whereas the number of "red" foods decreased substantially (School B: 10-2 per week; School C: 13-4). Canteen staff adjusted recipes to use more garden produce, although some packaged snacks persisted. Barriers included limited resources, irregular garden supplies, time constraints, and outside processed food vendors. Strong leadership and community participation elevated acceptability, but infrastructure and sustainability challenges persisted. Improving and integrating school gardens, improved canteen menus, and staff training can enhance school food environments in resource-limited schools. Success requires strong leadership, partnerships, practical planning, and sustained resources. Ongoing investment and collaboration are critical to building nutrition-friendly school environments that support students' health and well-being.

The Role of Intranasal and Inhaled Therapies in the Management of Cluster Headache: A Network Meta-analysis.

Aljeraisi Talal T

There are limited evidence-based nasal therapies available to treat cluster headaches. Likewise, there are not many head-to-head treatment comparisons available because most clinical trials have only compared therapies versus placebos. To compare different nasal therapies for cluster headache. We used four databases to search for records and then screened them for eligibility. In randomized studies, the risk of bias was assessed using the Cochrane tool for randomized clinical trials. The data were then retrieved and used in double-arm analyses using the R Software (free and open source). Zolmitriptan 10 mg and oxygen demonstrated the greatest efficacy for headache relief, while oxygen ranked highest for pain-free outcomes. Zolmitriptan 10 mg was most effective in reducing rescue medication use. Across outcomes, zolmitriptan 10 mg and oxygen consistently ranked as the most effective treatments, with no evidence of inconsistency. Intranasal therapeutic sprays expand the treatment options and may provide better results, more rapid relief, and higher patient satisfaction than oral or injectable medications. Although many intranasal therapeutic sprays showed variable efficacy in cluster headache relief, more randomized controlled trials on a large sample of patients with a longer follow-up duration are needed.

Health vulnerability behaviors among men who have sex with men in the context of mpox: a cross-sectional study, Brazil, 2022.

Santos Guilherme Reis de Santana GRS, Ribeiro Caíque Jordan Nunes CJN, Bittencourt Thamyres Vaz de Jesus TVJ, Barreto Nilo Manoel Pereira Vieira NMPV et al.

To analyze adherence to health vulnerability behaviors among men who have sex with men during the mpox public health emergency in Brazil. A cross-sectional, analytical study conducted in a virtual environment between September and December 2022 via an electronic questionnaire, with recruitment via social media/dating apps. The outcome of interest comprised four variables related to health vulnerability behaviors, and a multivariable analysis was conducted using a generalized linear model. A total of 1,449 men who reported having sex with men participated in the study, with a prevalence of health vulnerability behaviors of 13.9% (n=202), mainly due to not being vaccinated against mpox (50.4%), not adopting preventive care measures (30.6%), not undergoing testing (26.2%), and not following health treatment (9.9%). The multivariable model indicated a higher prevalence of the outcome independently associated with intimate contact with a person with mpox (adjusted prevalence ratio [APR]: 1.82; 95% confidence interval [95%CI] 1.33; 2.48), non-disclosure of serological status (APR 1.40; 95%CI 1.05; 1.87), and disbelief in mpox-related homophobia (APR 1.39; 95%CI 1.01; 1.90). The use of glory holes was associated with a lower prevalence (APR 0.43; 95%CI 0.20; 0.92). Greater health vulnerability was identified among participants who exhibited behaviors associated with a higher risk of mpox exposure, highlighting the need to plan targeted, population-based actions.

Frequent Social Media Use Associated With Tobacco, Alcohol, and Drug Use Among US High School Students.

Rumph Jelonia T JT, McKinnon Izraelle I II, Saelee Ryan R, Kiplagat Sandra J SJ et al.

The purpose of this study was to examine the associations between frequent social media use and tobacco, alcohol, and drug use among US high school students in grades 9 through 12. Data from the 2023 national Youth Risk Behavior Survey (N = 20 103) were analyzed to assess the associations between frequent social media use and current tobacco or nicotine use, current alcohol use, current binge drinking, ever marijuana use, ever prescription opioid misuse, and ever select illicit drug use. Frequent social media use was defined as using social media several times a day, about once an hour, or more than once an hour. Quasi-Poisson regression models estimated adjusted prevalence ratios (APRs) comparing substance use behaviors among students with frequent social media use versus infrequent or no social media use, overall and by sex. Models were adjusted for race and ethnicity and grade. Compared with students reporting infrequent or no social media use, students with frequent social media use were more likely to report current tobacco or nicotine use (APR = 1.88; 95% CI, 1.25-2.83), current alcohol use (APR = 2.07; 95% CI, 1.54-2.78), current binge drinking (APR = 2.55; 95% CI, 1.45-4.48), and ever marijuana use (APR = 2.09; 95% CI, 1.68-2.60). Associations were observed among female and male students. Frequent social media use was associated with several substance use behaviors among US high school students. Future research should examine mechanisms, including type of social media platform used, exposure to substance use-related content, and peer influences, to inform prevention strategies.

Laparoscopic Endometriosis Phenotypes and Preoperative Non-suspicion of Endometriosis in Women With Superficial Disease: A Retrospective Study in Kinshasa.

Monzango Sibo Guy Lambert M GLM, Itewa Monka Laetitia L

Background Superficial peritoneal endometriosis (SPE) is difficult to identify before surgery, especially without concomitant ovarian or deep disease. We described laparoscopic phenotypes and examined factors associated with the absence of documented preoperative suspicion of any endometriosis among women subsequently found to have SPE. Methods We retrospectively analyzed gynecologic laparoscopies performed at three Kinshasa sites between January 2019 and June 2026 by one surgeon. Superficial, ovarian, and deep phenotypes were recorded as non-mutually exclusive. The analytical outcome was absence of documented preoperative suspicion of endometriosis of any phenotype among women with laparoscopically identified SPE and classifiable preoperative records. Modified Poisson models with robust variance estimated exploratory adjusted prevalence ratios (aPRs). Results Of 1,743 gynecologic laparoscopies, 216 (12.4%; 95% CI, 10.9%-14.0%) showed endometriosis and 1,527 (87.6%) did not. SPE occurred in 139/216 (64.4%), ovarian disease in 137/216 (63.4%), and deep disease in 68/216 (31.5%); 104/216 (48.1%) had overlapping phenotypes. Among 135 women with SPE whose preoperative records were classifiable, 83 (61.5%; 95% CI, 53.1%-69.3%) lacked documented suspicion of endometriosis; this proportion was 46/56 (82.1%) in isolated SPE. In the clinical model, infertility was associated with higher non-suspicion (aPR 1.59; 95% CI, 1.07-2.35), while deep dyspareunia was associated with lower non-suspicion (aPR 0.65; 95% CI, 0.45-0.94). With additional adjustment for coexisting operative phenotypes, the infertility association weakened (aPR 1.39; 95% CI, 0.95-2.04) and the dyspareunia estimate remained borderline (aPR 0.70; 95% CI, 0.49-1.00). Conclusions SPE and ovarian disease were the most common laparoscopic phenotypes in this selected surgical cohort. An absence of documented suspicion of any endometriosis was frequent among women with SPE, especially isolated SPE. Associations with infertility and deep dyspareunia are exploratory and may reflect clinical documentation, surgical referral pathways, and coexisting disease. Prospective standardized assessments are needed.
