Osimertinib-Savolitinib in Advanced NSCLC after Progression during EGFR-TKI Therapy.

Lu Shun S, Girard Nicolas N, Yang James Chih-Hsin JC, Shepherd Frances A FA et al.

Savolitinib is an oral, potent, highly selective MET-tyrosine kinase inhibitor (TKI). Combination therapy with savolitinib and osimertinib, an epidermal growth factor receptor (EGFR)-TKI, has shown activity in patients with EGFR-mutated, advanced non-small-cell lung cancer (NSCLC) with MET overexpression, MET amplification, or both, after progression during EGFR-TKI therapy. In this international, phase 3, open-label trial, we randomly assigned, in a 1:1 ratio, patients with EGFR-mutated, locally advanced or metastatic NSCLC with MET overexpression (staining intensity of 3+ for MET in ≥90% of tumor cells on immunohistochemical analysis), MET amplification (≥10 gene copies on fluorescence in situ hybridization), or both whose disease had progressed during osimertinib therapy to receive osimertinib (at a dose of 80 mg once daily) plus savolitinib (300 mg twice daily) or intravenous platinum-pemetrexed. The primary end point was progression-free survival, as assessed by blinded independent central review. Secondary end points included overall survival and safety. Of the 1746 patients screened, 338 were randomly assigned to receive osimertinib-savolitinib (171 patients) or platinum-pemetrexed (167 patients). Osimertinib-savolitinib led to significantly longer progression-free survival than platinum-pemetrexed (hazard ratio for disease progression or death, 0.55; 95% confidence interval [CI], 0.42 to 0.72; P<0.001), as well as significantly longer overall survival (hazard ratio for death, 0.68; 95% CI, 0.50 to 0.91; P = 0.01; final analysis). The Kaplan-Meier estimate of 12-month progression-free survival was 36% in the osimertinib-savolitinib group and 10% in the platinum-pemetrexed group, and the estimate of 12-month overall survival was 70% and 53%, respectively. The most frequently reported adverse events that occurred during treatment in the osimertinib-savolitinib group were peripheral edema (in 49% of the patients), anemia (in 26%), and diarrhea (in 26%). Grade 3 or higher adverse events during treatment were reported in 64% of the patients in the osimertinib-savolitinib group and in 63% of those in the platinum-pemetrexed group. Osimertinib-savolitinib led to significantly longer progression-free and overall survival than platinum-pemetrexed among patients with EGFR-mutated, locally advanced or metastatic NSCLC with MET overexpression, MET amplification, or both that had progressed during osimertinib therapy (Funded by AstraZeneca; SAFFRON ClinicalTrials.gov number, NCT05261399.).

Healthcare use by people with dementia from diagnosis to the end of life: latent class and longitudinal analysis of linked healthcare data.

Yorganci Emel E, Stewart Robert R, Sampson Elizabeth L EL, Carr Ewan E et al.

The first year after diagnosis and the last year of life are critical periods for people with dementia. To (a) identify subgroups within a large sample of people with dementia, characterised by healthcare use in the year after diagnosis and the last year of life; (b) describe the biopsychosocial characteristics associated with these subgroups; and (c) explore moves between subgroups over time. A retrospective cohort of people diagnosed with dementia in London, UK, was assembled using linked clinical and administrative data-sets. We used latent class analysis to identify subgroups based on hospital and specialist mental healthcare service use in the year following diagnosis and the year before death. We used multinomial logistic regression to describe biopsychosocial characteristics and class membership, and latent transition analysis to explore moves between subgroups. Among 8623 people with dementia, we identified 3 subgroups of healthcare use in the year after diagnosis: 'low use' (49%), 'hospital use' (34%) and 'mental healthcare use' (17%). In the last year of life, four subgroups were identified: 'low use' (32%), 'moderate hospital use' (43%), 'high hospital use' (9%) and 'community healthcare use' (17%). Biopsychosocial factors, including age, ethnicity, and cognitive and physical problems, associated with subgroups differed for the time periods. Male gender, mixed dementia and vascular dementia diagnoses were associated with a higher risk of unplanned hospital use. Transition probabilities indicated unstable patterns of healthcare use after diagnosis and before death. Distinct subgroups and healthcare patterns in two care periods can facilitate proactive, needs-based service provision while potentially reducing emergency hospital use.

Economic impact of biomarker-based aging interventions on healthcare costs and individual value.

Felizzi Federico F

We investigate the economic impact of controlling the pace of aging through biomarker monitoring and targeted interventions. Using the DunedinPACE epigenetic clock as a measure of biological aging rate, we model how different intervention scenarios affect frailty trajectories and their subsequent influence on healthcare costs, lifespan, and health quality. Our model demonstrates that controlling DunedinPACE from age 50 onward can reduce frailty prevalence, resulting in cumulative healthcare savings of up to CHF 131,608 per person over 40 years in our most optimistic scenario. From an individual perspective, the willingness to pay for such interventions reaches CHF 3.2 million in the most optimistic scenario when accounting for both extended lifespan and improved health quality. When the direct cost of epigenetic testing is included, the healthcare-system case is sensitive to test price and testing frequency, whereas the individual-value case remains strongly positive. These findings suggest substantial economic value in technologies that can monitor and modify biological aging rates, providing evidence for both healthcare systems and consumer-focused business models in longevity medicine.

Conductive Gels for Wearable and Attachable Healthcare Devices: Materials, Sensing Mechanisms, Interfaces and Applications.

Jeong Jinmo J, Jung Dongwuk D

Wearable and attachable healthcare devices provide valuable insights across a broad range of applications, from health monitoring and disease diagnosis to the assessment of daily human activities. For these devices, both the formation of high-performance electrodes and mechanical, electrical, and physiological stability at human-device interfaces are critically important. Conductive gels, which combine deformable polymer networks with electrical conductivity, show considerable potential as functional materials for such applications. However, existing reviews have often considered material properties, interfacial behavior, and device reliability separately. This review examines hydrogels, ionogels, and organogels by linking material design, stimulus- and analyte-responsive sensing, biological and device interfaces, and healthcare applications. This integrated perspective emphasizes evaluating conductive gels within complete healthcare devices, where material properties, biological interfaces, and device integration jointly determine practical performance. Such evaluation needs to address signal quality and the stability of the assembled system during deformation, environmental exposure, and prolonged use. Future progress will require reproducible device fabrication and standardized validation tailored to the intended application, with systematic assessment of long-term biosafety and alignment with relevant regulatory requirements. Integration with flexible electronics, intelligent data processing, and feedback control may further enable reliable and responsive healthcare platforms.

Workroom contact networks: implications for COVID-19 and other respiratory infections.

Hanson Alec W AW, Evans Nicholas J NJ, Pemmaraju Sriram V SV, Ossai Michael-Sean MS et al.

To examine how physician workroom size affects healthcare professional contact networks and the risk of respiratory virus transmission. Prospective observational study with agent-based simulations. Two physician workrooms of differing size (internal medicine, 208.7 sq ft; cardiology, 112.6 sq ft) in a large academic hospital. We deployed wireless sensor networks over 18 days in internal medicine and 14 days in cardiology workrooms. Physicians, residents, and medical students wore proximity-sensing badges during work shifts. Contacts were defined as two healthcare professionals within six feet, based on calibrated received signal strength indicators. Contact networks were generated in one-minute intervals. We simulated respiratory disease spread using a Susceptible-Infected (SI) model across varying transmission probabilities and estimated infection risk at different room densities. Average occupancy was 2.7 persons in internal medicine and 2.3 in cardiology. Despite lower occupancy, the smaller cardiology room exhibited greater contact density: healthcare professionals in the smaller room had higher mean and median numbers of contacts. SI simulations showed consistently higher risk of secondary infections in the cardiology room, even at lower transmission probabilities. Workroom size significantly affects healthcare professional contact density and infection risk. Smaller rooms increased opportunities for transmission despite lower average occupancy. Designing larger workrooms, limiting occupancy, or reconfiguring shared spaces may reduce infection transmission in healthcare facilities.

Knowledge, Attitudes, and Practices Regarding Marburg Virus Disease Among Healthcare Workers in Dilla University Teaching Hospital, Ethiopia: A Cross-Sectional Study.

Demeke Abel Desalegn AD, Tamir Takla T, Abebe Mesfin M, Endawoke Migbaru M et al.

Healthcare workers are a first contact for patients with infectious diseases and may be at occupational risk during hemorrhagic fever outbreaks. The Marburg virus disease (MVD) outbreak in southern Ethiopia, ongoing during data collection in December 2025, highlighted the need to assess healthcare workers' knowledge, attitudes, and practices regarding MVD. Therefore, this study assessed knowledge, attitudes, and practices and examined factors associated with practice among healthcare work. A cross-sectional study of 238 healthcare workers at Dilla University Teaching Hospital was conducted from December 1 to December 22, 2025. Descriptive statistics, frequencies, and percentages summarized participant characteristics and KAP responses. Binary logistic regression examined factors associated with good practice. Variables with p value ≤ 0.25 in bivariable analysis were included in the multivariable model. Adjusted odds ratios (AORs) with 95% confidence intervals (CIs) were calculated, and significance was set at p < 0.05. About 51.7%, 57.1%, and 55.0% of participants had good knowledge, positive attitudes, and good practice toward MVD, respectively, based on study-specific cutoffs. Most identified direct contact with blood and body fluids as an MVD transmission route (92.0%). However, gaps were observed regarding environmental risk factors and MVD distribution and origin. Most recognized reporting suspected cases (91.2%), avoiding contact with infected individuals or their possessions (91.2%), and using personal protective equipment (87.8%). In multivariable analysis, modification of working habits from concern about acquiring MVD (AOR = 2.19, 95% CI: 1.21-3.99) and good MVD knowledge (AOR = 1.96, 95% CI: 1.13-3.39) were positively associated with good practice. Findings indicate gaps in MVD-related knowledge and infection prevention among healthcare workers at Dilla University Teaching Hospital. Targeted competency-based training, simulation, IPC resources, and facility-level preparedness systems are warranted to strengthen readiness for MVD and other viral hemorrhagic fever events.
