Divergent Human Papillomavirus Genotype Spectra in Three Indigenous Communities of Venezuela: Implications for Vaccine Choice and Cervical Screening.

Garcés María Fátima MF, Guzmán Silva Jefferson J, Aranguren Daniel D, Figueira Lina L et al.

Background: Genotype-resolved data on human papillomavirus (HPV) are almost absent for Venezuelan indigenous women. Methods: We studied 260 women from La Guajira (Wayuu, n = 94), Maniapure (Eñepa, n = 141), and Delta Amacuro (Warao, n = 25), processing all self-collected samples with a single real-time PCR assay that resolves 21 genotypes; cytology was available for 144. Results: HPV prevalence was 55.4% (95% CI 49.3-61.3) and high-risk prevalence was 51.2% (95% CI 45.1-57.2), rising from 42.6% in La Guajira to 59.6% in Maniapure and 80.0% in Delta Amacuro (p = 0.001). Nineteen genotypes were detected. HPV-31 was most frequent (15.8%), followed by HPV-16 (11.5%), and each community had its own profile, with HPV-68 and HPV-56 leading in Delta Amacuro. Half of the infected women carried more than one type. Cytology and HPV testing overlapped little: 20 of 78 HPV-positive women with a smear (25.6%) had an abnormal result (κ = 0.056). At least one nonavalent vaccine type was present in 69.4% of infected women, compared with 30.6% for the bivalent vaccine. Conclusions: HPV vaccination is not part of the Venezuelan national program; these data favor the nonavalent formulation, and HPV-based screening would need a feasible triage pathway for the many women who test positive.

Meeting report of the fifth symposium organized by the coalition to strengthen the HPV immunization community (CHIC): Nairobi, Kenya - advancing sustainable HPV vaccination programs in Africa.

Bolio Ana A, Jennings Mary Carol MC, Burdier Ricardo R, Jindal Sakshi S et al.

HPV vaccination coverage across Africa has improved substantially in recent years but remains below elimination thresholds, with persistent gaps in reaching hard-to-reach populations and growing uncertainty around future financing. This report summarizes proceedings from the fifth regional symposium of the Coalition to Strengthen the HPV Immunization Community (CHIC), held in Nairobi, Kenya, in October 2025 in collaboration with Kenya's Ministry of Health. The three-day symposium convened over 100 participants from 22 African countries, including national immunisation program managers, ministry officials, researchers, civil society representatives, and global partners (WHO, UNICEF, Gavi, PATH, the Gates Foundation). The agenda, developed through stakeholder interviews and prior scientific and advisory input, centered on six subthemes related to program sustainability. Plenary sessions addressed epidemiological trends, national policy updates including Kenya's transition to a single-dose schedule, and implications of Gavi's upcoming consolidated funding model (Gavi 6.0). Country snapshot presentations and service-delivery case studies facilitated cross-country peer learning. On day three, though an electronic poll of helped to prioritize 12 candidate implementation themes, which structured subsequent round table working groups. Countries then developed individual action plans identifying priority challenges, responsible stakeholders, and implementation timelines. Sustainable service delivery, financing, and data systems emerged as the highest-priority implementation themes. Country action plans most frequently targeted data systems strengthening, communication and community engagement, sustainable service delivery, and financing mechanisms, with additional attention to service integration, hard-to-reach populations, and target population expansion. Cross-cutting synthesis identified recurring concerns regarding HPV vaccination for people living with HIV, fragmented data systems, the role of program integration, financing sustainability under evolving donor models, and underrecognized risks from misinformation, including emerging AI-generated content. The symposium reinforced CHIC's role as a platform for peer-to-peer learning across countries at varying stages of HPV vaccine introduction, surfacing shared structural barriers to sustainability alongside country-specific action plans. Findings underscore the need for sustained political commitment, financing transition planning, digital system strengthening, and multisectoral integration to support the WHO goal of cervical cancer elimination by 2030.

Human Papillomavirus (HPV) Infection-Associated Epithelial-to-Mesenchymal Transition: Implication in Tumorigenesis and Pathogenesis of HPV-Positive Cancers.

Kabakov Alexander E AE, Matchuk Olga N ON, Khokhlova Anna V AV, Kaprin Andrey D AD

Epithelial-to-mesenchymal transition (EMT) is a programmed and reversible process that allows epithelial cells to temporarily acquire a mesenchymal cell phenotype with increased migratory and invasive capacity. Analogous phenotypic modulation can also occur in carcinoma cells, which, after undergoing EMT, exhibit cancer stem cell-like properties and actively migrate, leading to tumor invasion and metastases. This structured narrative review focuses on the molecular crosstalk between human papillomavirus (HPV) and EMT, which occurs in HPV-infected human epithelium and in HPV-positive cancers of different location (cervical, head and neck, colorectal, lung, and others). The main questions addressed here are: (i) how HPV-driven EMT may contribute to the malignant transformation of HPV-infected epithelial cells, (ii) what mechanistic links may exist between the HPV positivity and EMT in human cancers, and (iii) how EMT in various HPV-positive human tumors may influence their pathogenesis, treatment modalities and outcomes. The relationship between HPV infection and EMT program is delineated as a complex multicomponent mechanism triggered by exogenous and endogenous stimuli and involving specific ligand-receptor interactions, membrane channels, transcription factors, signaling pathways, epigenetic regulators, etc. The review also discusses some of the potential applications of EMT-related biomarkers and molecular targets in the prevention, prediction and treatment of HPV-induced cancers.

The Relationship Between Interferon Status and HPV Infection in Patients with Oral Epithelial Dysplasia: An Observational Study.

Khisamieva Guzel G, Sologova Diana D, Svitich Oxana O, Diachkova Ekaterina E et al.

Background/Objectives: Oral potentially malignant diseases (OPMDs) with signs of epithelial dysplasia are characterized by a high risk of malignancy. However, there are still no algorithms for their early diagnosis and prediction, due to the many possible predisposing factors, including human papillomavirus (HPV) infection and decreased immunity. There is a need to assess the relationship between the manifestations of oral epithelial dysplasia (OED), HPV invasion in the oral cavity, and interferon status to improve the identification and characterization of patients with different degrees of dysplasia. Methods: This study was designed and reported with reference to the STROBE checklist. The study aimed to assess the association between interferon status and HPV invasion in patients with OED. As per the inclusion and exclusion criteria, this observational study included 180 patients with histologically confirmed oral epithelial dysplasia associated with leukoplakia or oral lichen planus. Interferon (IFN) status was assessed by measuring serum interferon activity, spontaneous interferon production, IFN-α/β, and IFN-γ activity in peripheral blood. HPV DNA was detected and genotyped in biopsy specimens using polymerase chain reaction. Results: HPV-positive patients demonstrated significantly lower IFN-α/β and IFN-γ activity compared with HPV-negative patients (p < 0.001). Severe OED was more frequently observed in HPV-positive individuals, whereas mild dysplasia predominated among HPV-negative patients (p < 0.05). Interferon activity progressively decreased with increasing dysplasia severity, with the lowest IFN-α/β and IFN-γ activity detected in patients with severe dysplasia (p < 0.001 and p < 0.05, respectively). A significant positive correlation between IFN-α/β and IFN-γ activity was identified in patients with moderate dysplasia (p = 0.003). Multivariate binary logistic regression analysis demonstrated that HPV positivity (adjusted OR = 4.62, 95% CI: 2.18-9.79; p < 0.001) and reduced IFN-α/β activity (adjusted OR = 1.34 per 10-unit decrease, 95% CI: 1.14-1.58; p < 0.001) were independently associated with severe dysplasia. The multivariable model demonstrated acceptable discriminatory ability for identifying patients with severe dysplasia (AUC = 0.736), with a sensitivity of 89.4% and a specificity of 56.1%. Conclusions: The findings suggest that HPV-associated OED is linked to reduced interferon status, particularly lower IFN-α/β and IFN-γ activity in more severe dysplastic lesions. Combined assessment of interferon status and HPV detection may help identify patients at higher risk of severe OED.

Development of an HPV Self-Collection Brochure for Unhoused Individuals Using Learner Verification and Revision.

Morenz Anna M AM, Inzunza-Uysal Dina J DJ, Robles-Morales Rogelio R, Mockbee Joy J et al.

Individuals who are unhoused face disproportionately high rates of cervical cancer incidence and multiple barriers to screening, yet tailored educational materials to support human papillomavirus (HPV) self-collection in this population remain scarce. To address this gap, we conducted a sequential, two-phased qualitative study using Learner Verification and Revision methodology to iteratively develop and refine an instructional brochure on HPV self-collection designed for, and with, individuals who are unhoused. We first conducted focus groups with community health educators (n = 6) and street medicine team members (n = 12), followed by semi-structured interviews with individuals who were unhoused (n = 19) and had participated in an HPV self-collection pilot study. Data collection assessed comprehension, attraction, cultural-linguistic acceptability, and persuasion of the brochure, with trauma-informed care as an additional domain, and transcripts were analyzed using thematic analysis. Participants broadly found the brochure clear, visually appealing, and easy to follow, while identifying key areas for refinement, including greater brevity, more inclusive visuals, regional Spanish vocabulary adaptation, and language modifications to strengthen trustworthiness and empowerment. Street medicine team members emphasized that the brochure best complements verbal counseling rather than serving as a standalone resource. This community-engaged, iterative process yielded a tailored, trauma-informed educational tool which is publicly available and editable. As HPV self-collection expands across the United States, such materials will be valuable to support equitable access to cervical cancer screening.

Bridging the gap: advanced chromatin biotechnologies for circadian epigenomic profiling in head and neck squamous cell carcinoma.

Khan Sehrish S, Said Raed R, Eskiw Christopher C, Thuot François F et al.

Surgery, radiotherapy and chemotherapy remain the main therapies for treating head and neck cancers, however new modalities have emerged with an emphasis to understand the complexity of the tumor biology. One emerging layer of complexity is the circadian clock, disrupted in many cancers, contributes to pathogenesis and progression due to its crosstalk with cancer pathways. While transcriptional alterations of core clock genes have been described in Head and Neck squamous cell carcinoma (HNSCC), the higher-order epigenetic and chromatin-based mechanisms underlying circadian dysregulation remain poorly defined. In this review, we highlight the potential of several chromatin-based biotechnologies in characterizing circadian clock disruption and identifying epigenetic biomarkers in HNSCC. Powerful assays including Chromatin conformation capture, Chromatin immunoprecipitation, DNA methylation profiling, ATAC-seq and single cell technologies can dissect circadian disruption at multiple regulatory layers. These approaches enable genome-wide mapping of chromatin accessibility, histone modifications, and 3D chromatin organization. Integrating these approaches across HPV-positive (HPV+) and HPV-negative (HPV-) HNSCC provides a framework to determine whether circadian disruption reflects transcriptional deregulation alone or involved structural genome remodeling that actively drives tumor progression. Linking circadian disruption to epigenomic reprogramming in HNSCCs can serve to identify mechanisms of therapeutic resistance and identify vulnerabilities. Advancing chromatin-informed circadian biomarker discovery in HNSCC has the potential to shift precision oncology from static genomic profiling toward dynamic, architecture-based epigenetic regulation.
