Bone-targeting engineered exosomes delivering teriparatide mRNA for the treatment of osteoporosis.

Wang Diaodiao D, Sun Yuyang Y, Sun Wenqiang W, Zhang Shudong S et al.

Teriparatide (TPTD) is an established osteoanabolic agent for the treatment of osteoporosis; however, its short circulating half-life necessitates repeated administration, while limited skeletal selectivity may reduce delivery efficiency. To address these limitations, we developed an Asp6-functionalized, bone-targeting exosome system carrying TPTD mRNA (Asp6-Exo@TPTD). An Exo-Motif sequence was incorporated into the TPTD mRNA construct to facilitate hnRNPA2B1-associated enrichment of the transcript in exosomes, while surface modification with the hydroxyapatite-binding peptide Asp6 was used to enhance skeletal affinity. BMSC-derived exosomes may additionally provide intrinsic regenerative signals that complement the osteoanabolic activity of TPTD. Mouse bone marrow mesenchymal stromal cells (BMSCs) were incubated with different exosome formulations to evaluate intracellular TPTD mRNA transfer, TPTD protein production, CRE-dependent reporter activity, and osteogenic differentiation. Free TPTD was included as a pharmacological reference in the functional, osteogenic, and animal efficacy experiments. The bone-binding properties of the engineered exosomes were assessed using hydroxyapatite-containing bone samples and ex vivo bone tissues. In vivo biodistribution was evaluated by fluorescence imaging after intravenous administration. Therapeutic efficacy was investigated in an ovariectomy-induced osteoporosis mouse model by assessing trabecular microarchitecture, histological changes, serum bone-formation markers, and dynamic bone formation. Preliminary tolerability was evaluated by major-organ histology and serum biochemical analyses, including serum calcium and hepatic and renal function indices. Asp6-Exo@TPTD increased intracellular TPTD mRNA levels and TPTD secretion in recipient BMSCs and enhanced CRE-dependent luciferase activity, osteogenic gene expression, alkaline phosphatase staining, and extracellular matrix mineralization. Asp6-Exo@TPTD exhibited greater binding to bone tissues than both unloaded Exo and non-targeted Exo@TPTD in vitro and ex vivo. Following systemic administration, bone-associated fluorescence remained detectable for at least 72 h, with greater femoral and tibial fluorescence and lower relative hepatic fluorescence than those observed with unmodified exosomes. In ovariectomized mice, weekly administration of Asp6-Exo@TPTD improved trabecular microarchitecture, increased serum procollagen type I N-terminal propeptide levels, and enhanced the mineral apposition rate. No overt histopathological abnormalities or significant alterations in serum calcium or routine hepatic and renal biochemical indices were detected. Asp6-functionalized exosomes carrying TPTD mRNA promoted osteogenic activity, enhanced bone-associated distribution, and attenuated ovariectomy-induced bone loss. These findings support Asp6-Exo@TPTD as a promising exosome-based strategy for bone-targeted TPTD mRNA delivery and osteoporosis treatment.

Medication-related osteonecrosis of the jaw: Current concepts in epidemiology, pathophysiology, diagnosis, and management.

Łodygowski Oskar O, Antoń Bartłomiej B, Kozakiewicz Marcin M, Trybek Grzegorz G et al.

Aim: The aim of this review was to highlight the ongoing challenge of medication-related osteonecrosis of the jaw (MRONJ) in everyday clinical practice. Materials and Methods: This review was conducted following a structured narrative approach, utilizing PubMed as the primary research database. The following keywords and their combinations with Boolean operators (AND, OR) were used: MRONJ, osteonecrosis, bisphosphonates, bone density conservation agents, denosumab. The literature search spanned publications from 2003 to April 2025, corresponding to the first description of bisphosphonate-related osteonecrosis of the jaw by Marx and subsequent evolution of the diagnostic and therapeutic concepts. The final set of 32 articles were reviewed and synthesized narratively. Conclusions: Management strategies are examined across disease stages, from conservative measures and pharmacologic interventions to surgical resection and reconstruction. Emerging therapies, including teriparatide, hyperbaric oxygen, platelet-rich preparations, and cryosurgery, show promise as adjunctive treatments. Prevention remains paramount, emphasizing pre-treatment dental evaluation, patient education, and interprofessional collaboration.

Emerging therapeutic strategies for medication-related osteonecrosis of the jaw: From microenvironmental modulation to precision medicine (Review).

Zheng Xiao-Lu XL, Zhan Xiao-Zhen XZ, Liu Jin-Jin JJ, Leng Chang-Yun CY et al.

Medication-related osteonecrosis of the jaw (MRONJ) is a challenging complication associated primarily with antiresorptive and antiangiogenic therapies. Its pathogenesis involves a complex interplay of impaired bone remodeling, angiogenesis, inflammation and infection, immune regulation and soft tissue healing. Established management includes conservative care, antimicrobial and symptomatic treatment when indicated and surgical intervention, however clinical outcomes remain variable. The present review aimed to summarize pharmacological, regenerative, biomaterial-based and physical or adjunctive strategies targeting the pathological microenvironment of MRONJ, with emphasis on their mechanisms of action and clinical evidence. The present review also examines the potential role of precision medicine in individualized risk assessment and treatment. Teriparatide, pentoxifylline/tocopherol, autologous platelet concentrates and human amniotic membrane have varying degrees of clinical support, although evidence remains limited or heterogeneous for several approaches. By contrast, geranylgeraniol, hydrogels, most mesenchymal stromal cell-based or cell-derived strategies, natural products and melatonin are supported predominantly by preclinical or limited clinical evidence. Other adjunctive approaches, including hyperbaric oxygen, ozone therapy, photobiomodulation, laser-assisted surgery and antimicrobial photodynamic therapy, require further validation. Overall, these strategies may complement established management, but their clinical readiness varies. Standardized clinical studies and integration of multidimensional patient data may facilitate more robust evaluation and individualized MRONJ management.

Prevention and treatment of osteoporosis in postmenopausal women.

Stevenson John J

Postmenopausal osteoporosis is a major public health concern characterised by rapid loss of bone mineral density due to estrogen deficiency, leading to increased risk of fragility fractures. This article reviews current clinical strategies for the prevention and treatment of the condition in postmenopausal women. Primary prevention discusses lifestyle modification, including adequate dietary intake of calcium and vitamin D, regular weight-bearing exercise and cessation of smoking and excess alcohol consumption. For women at elevated fracture risk, pharmacological interventions are essential. Therapies include hormone replacement therapy (HRT), bisphosphonates, Selective Estrogen Receptor Modulators (SERMS) and RANKL inhibitors like denosumab, which function as antiresorptive agents to slow bone breakdown. For those with high fracture history, anabolic agents such as teriparatide or romozosumab are utilised. Early identification via personalised risk assessment, dual-energy X-ray absorptiometry (DEXA) screening and treatment where indicated is essential. Combining adjunctive lifestyle modification with pharmacological intervention offers a framework to maintain skeletal health, preserve independence and enhance long-term quality of life post menopause.

Refractory hypoparathyroidism in paediatric autoimmune polyglandular syndrome type 1: successful long-term use of teriparatide.

Perez Carolina C, Adarme Nicolas N, Hernandez Catalina C, Sarmiento Maria Paula MP

Autoimmune polyglandular syndrome type 1 (APS-1) is a rare monogenic disorder caused by pathogenic variants in the autoimmune regulator (AIRE) gene. It typically manifests in childhood with at least two features of the classic triad: chronic mucocutaneous candidiasis, hypoparathyroidism, and adrenal insufficiency. We present the case of an 11-year-old girl from a consanguineous family, with severe hypocalcaemia and chronic oral candidiasis. Despite high-dose calcium and calcitriol therapy, she experienced recurrent symptomatic hypocalcaemia requiring multiple hospitalisations. Genetic testing confirmed homozygosity for the pathogenic AIRE variant c.967_979del, establishing the diagnosis of APS-1. Due to poor biochemical control, teriparatide (parathyroid hormone, PTH [1-34]) was initiated at 20 g twice daily, leading to rapid improvement in calcium-phosphate homeostasis, a 75% reduction in oral calcium requirements, and no further hospitalisations over a two-year period. The therapy was well tolerated with no adverse events. This is the first paediatric case reported in Colombia using recombinant PTH (1-34) for APS-1-related hypoparathyroidism, supporting its therapeutic role in refractory cases.
