Trauma scoring systems for evaluating trauma center triage criteria in older adults: a retrospective cohort study.

Sulej-Niemiec Małgorzata M, Kopta Andrzej A, Żurowska-Wolak Magdalena M, Kręglicki Łukasz Ł et al.

Standardized trauma assessment is essential for early clinical decision-making; however, it remains challenging in older adults because age-related physiological changes may obscure injury severity. Accurate triage is critical to ensure timely access to trauma centers (TCs). While triage decisions must rely on immediately available clinical findings, advanced trauma scoring systems are typically based on diagnostic data obtained later in the course of care. Although these tools are not suitable for real-time triage, they are established predictors of mortality and may provide an objective basis for evaluating TC qualification criteria. Because current national TC qualification criteria do not adequately account for age-specific factors, they may contribute to undertriage in older patients. This study aimed to retrospectively identify the trauma scoring system most suitable for evaluating TC qualification criteria in geriatric trauma patients. We conducted a retrospective cohort study of patients admitted to the emergency department of a single clinical hospital with a TC between 2017 and 2020. Among 155,320 ED admissions, 6,541 trauma patients aged ≥ 65 years were identified. Injury severity and physiological status were assessed using the coded Revised Trauma Score (RTSc), Injury Severity Score (ISS), and Trauma and Injury Severity Score (TRISS). A predefined subgroup of patients meeting national TC qualification criteria (TC group 65+) was analyzed separately. Predictive performance for in-hospital mortality was evaluated using receiver operating characteristic (ROC) curve analysis. TRISS showed the highest discrimination for in-hospital mortality (AUC 0.888), followed by ISS (0.867) and RTSc (0.687). In the TC group 65+, the mean TRISS-predicted survival probability was 34.58%, whereas observed in-hospital survival was 48.72%. The estimated undertriage rate was 73.47%, suggesting that current national TC qualification criteria may perform inadequately in older trauma patients. Current national TC qualification criteria may contribute to undertriage among older patients, potentially delaying access to advanced trauma care. Among the evaluated scoring systems, TRISS demonstrated the highest discriminatory performance for in-hospital mortality and appears to be the most suitable retrospective reference standard for assessing TC qualification criteria in geriatric trauma patients.

Computational screening of FeM@g-C6N6 (M = selected 3d-5d transition metals) diatomic catalysts for selective electrocatalytic nitrogen reduction.

Sa Rongjian R, Wu Liangchun L, Hu Linfeng L, Chen Xintian X et al.

Electrocatalytic nitrogen reduction (NRR) offers a sustainable route for ammonia synthesis at room temperature, but is limited by sluggish N2 activation and the competing hydrogen evolution reaction (HER). We systematically evaluated 18 FeM@g-C6N6 (M = V, Cr, Mn, Co, Ni, Cu, Nb, Mo, Tc, Ru, Rh, Pd, Ta, W, Re, Os, Ir, and Pt) diatomic catalysts using density functional theory (DFT) calculations. Through stepwise screening based on stability, N2 adsorption strength, HER suppression, and first-step hydrogenation free-energy changes, 12 promising candidates were identified. Among them, Fe-EB exhibited the best performance via a distal pathway, with a limiting potential of -0.44 V, followed by Tc-ES (-0.45 V) and Ta-SD (-0.46 V). Electronic structure analysis indicates that N2 activation stems from electron back-donation to the π* orbital and orbital hybridization between N p orbitals and the metal d orbitals. Descriptor analysis further reveals that the degree of N2 activation can be rationalized by the metal d-band center, which correlates linearly with the NN ICOHP (R2 = 0.93), and by the charge donated to N2, which scales with the NN bond elongation (R2 = 0.92). Adjacent *OH co-adsorption, used to approximate a hydroxylated local adsorbate environment, influences the reaction pathways and activity of the catalyst but does not alter its fundamental activation mechanism. This study provides useful mechanistic insights and descriptors for the rational design of g-C6N6-based diatomic NRR catalysts.

Investigating the Effects and Mechanism of Phytosterols in Reducing Liver Cholesterol in L02 Cells and Golden Hamsters.

Wang Ruxia R, Hu Linfang L, Tu Yue Y, Zhai Zhenya Z et al.

This study examines the cholesterol-lowering effects of soybean sterols (SS) in L02 cells and hamsters via SREBP-2/LXRα. Using L02 cells and hamsters as research subjects, we conducted an analysis on the impact of soybean sterols (SS) on the assimilation, synthesis, and elimination of cholesterol from hepatic cells. Through metabolomic profiling, we aimed to elucidate the mechanism by which soybean sterols lower cholesterol levels. SS orchestrated a reduction in total cholesterol (TC) and triglyceride (TG) levels through the regulation of genes associated with cholesterol absorption and biosynthesis in L02 cells. In hamsters, SS supplementation led to a dosage-dependent decrease in TC and free cholesterol (FC) in serum and livers. Furthermore, the potential mechanism underlying SS-induced cholesterol lowering may involve modulation of metabolites such as StE (22:6), PE (29:1/18:2), and ZyE (18:3). Interestingly, it was found that SS had a minimal impact on TG levels, but excessive SS led to an increase in TG levels. The discoveries underscore the advantageous influence of phytosterols on hepatic cholesterol metabolism; however, given the effects of phytosterols on TG, caution should be exercised in the intake dosage. This research lays down a fundamental cornerstone for the prevention and treatment of elevated cholesterol levels. This study provides a mechanistic basis for the cholesterol-lowering benefits of soybean-derived phytosterols, demonstrating their direct role in regulating hepatic cholesterol synthesis and excretion. The findings support the inclusion of phytosterols in functional foods or dietary supplements for managing hypercholesterolemia. However, the observed dose-dependent effect on triglycerides-with the highest dose increasing TG levels-serves as a critical translational caution. It underscores that while phytosterols are effective for cholesterol reduction, excessive intake beyond recommended dietary guidelines (approximately 2 g/day in humans) may have unintended metabolic consequences. This research informs both the development of phytosterol-enriched products and the establishment of safe intake recommendations for cardiovascular health promotion.

e MolFrag2 and eSynth2: Sound Fragment-Based Molecular Decomposition and Reconstruction.

Bess Adam A, Rowland Sean S, Alvin Chris C, Adibifard Meisam M et al.

We present eMolFrag2 and eSynth2, enhanced successors to eMolFrag and eSynth for fragment-based molecular decomposition and reconstruction. eMolFrag2 is a complete reimplementation of eMolFrag in Python, using RDKit and graph-based molecular representations to preserve connectivity, identify free atoms, combine adjacent linkers, and avoid aromaticity ambiguities that impair downstream reconstruction. eSynth2 generalizes fragment recombination by removing hard-coded molecular-property filters, supporting multiple bonds to a single brick atom, and adding a targeted reconstruction mode for soundness validation. Across 36,534 nonredundant DUD-E active compounds, the eMolFrag2 → eSynth2 workflow reconstructed every input molecule exactly (Tanimoto coefficient, TC = 1.0), whereas the original eMolFrag→ eSynth workflow reconstructed 51 of the 100 molecules in a benchmark subset. The upgraded workflow also produced broader recombinant chemical spaces, including higher entropy in a four-molecule diversity analysis (2.78 versus 2.70). Both tools are open-source, platform-agnostic, and available for academic and commercial use.

Electromagnetic bronchoscopy versus computed tomography for pulmonary nodule localization.

Csaba Márton M, Megyesfalvi Zsolt Z, Salamon Anita A, Andi Judit J et al.

Electromagnetic navigational bronchoscopy (NAVIB) and CT-guided transthoracic marking (CTI) are two possible methods for preoperative marking. We have conducted a study to determine whether one technique performs better in a selected patient population. In a single-center, 1:1 randomized controlled trial we compared NAVIB using indocyanine green and CTI with Technetium-99m during uniportal wedge resections of lung nodules. Primary endpoint was the duration of surgery. Secondary endpoints included wedge resection time, complication rates, surgical margin, and surgeon satisfaction questionnaires. 63 patients were randomized, with 32 assigned to the NAVIB and 31 to the CTI group. Surgeries' duration was similar, with a median of 52.5 minutes (IQR:40-70) for NAVIB and 45 minutes for CTI (IQR:30-60;p= 0.664). The median wedge time for NAVIB was 15.5 minutes (IQR: 13-20), while for CTI it was 12 minutes (IQR: 9-15;p=0.0007). Grade II or higher complication rates were 9.4% for NAVIB vs 12.9% for CTI; (p = 0.144) and lesion's distance to staple line was 5.7mm (CI:4.2-7.2) vs. 7.0mm (CI:4.3-9.9) respectively (p = 0.355). In the questionnaire from 0 (not helpful) to 3 (very helpful), CTI received a median score of 3 (IQR;3-3), while NAVIB's was 2.5 (IQR;2-3;p = 0.045). After Bonferroni correction, questionnaire differences were not significant. The randomized trial failed to demonstrate a significant difference in overall surgery duration between electromagnetic navigational bronchoscopy and computed tomography-guided transthoracic marking.

Auricularia auricula-judae Peptides Modulate Lipid Metabolic Disturbances and Gut Microbiota Composition in High-Fat-Diet-Fed Mice.

Yang Feiyu F, Tao Jia J, Li Xin X, Sun Jing J et al.

High-calorie diets and sedentary lifestyles have led to a rising global prevalence of obesity, which constitutes a serious publichealth challenge. In this study, we investigated the antiobesity effects and underlying molecular mechanisms of Auricularia auricula-judae peptides (AAP) in high-fat-diet-induced obese mice. AAP intervention markedly restrained bodyweight gain. At the highest dose, the final body weight of AAP-treated mice was approximately 20% lower than that of the untreated high-fat-diet group. AAP reduced plasma and hepatic triglyceride (TG) and total cholesterol (TC) levels with a dose-dependent tendency and restored the histological morphology of liver and epididymal white adipose tissue. AAP mediated reshaped gut microbiota composition, improved intestinal homeostasis, and mitigated hepatic chronic inflammation. Moreover AAP significantly elevated fecal total bile acid (TBA) content and up-regulated the hepatic mRNA expression of lipolysis-related genes Cpt1, Ppar-α, Atgl and cholesterol catabolic gene Cyp7a1 thereby restoring hepatic lipid metabolic homeostasis. Collectively these findings demonstrate that AAP exerts promising antiobesity activity in obese mice and has great potential for development as functional food ingredients.
