[An interdisciplinary approach to hypertension management].

Hypertension is a major public health issue affecting nearly one billion people worldwide, including 1.5 million in Switzerland. Despite the availability of effective treatments, blood pressure control remains suboptimal, with many patients failing to achieve recommended targets. This is partly explained by therapeutic inertia, suboptimal medication adherence, and limited health literacy. Scientific societies increasingly advocate for a comprehensive, patient-centred care model involving all healthcare professionals who can contribute to blood pressure control, including primary care physicians, pharmacists, and specialized nurses. This article reviews the rationale for such an approach, its implementation, and the complementary roles of the healthcare professionals involved.

Point-of-care echocardiography screening for hypertrophic cardiomyopathy using automated deep-learning analysis.

Hypertrophic cardiomyopathy (HCM) remains underdiagnosed due to limited access to expert imaging. We developed and validated a deep-learning (DL)-based echocardiographic model adaptable to point-of-care ultrasound (POCUS) for scalable HCM screening. We retrospectively analysed 134 956 expert transthoracic echocardiograms (TTE) from 73 598 patients at Sheba Medical Center (2007-2022). A TTE-trained DL model integrating structural features and temporal motion patterns from parasternal long-axis and apical four-chamber views estimated HCM probability. Performance was evaluated in an independent test cohort and clinical subgroups. External validation used bedside POCUS studies from non-cardiologists with handheld devices. The test cohort included 12 096 patients with 119 confirmed HCM cases (prevalence 0.98%; median age 75 years, 57% male). HCM-positive patients showed increased expert TTE-measured septal (1.67 [1.5, 2.0] vs. 1.01 [0.9, 1.19] cm) and posterior wall thickness (1.1 [1.0, 1.3] vs. 0.9 [0.8, 1.0] cm) (P < 0.001). The model achieved excellent discrimination with an area under the curve of 0.982 (95% CI 0.966-0.993), sensitivity 88.2%, and specificity 97.3%, robust across subgroups. The POCUS cohort (n = 1047, median age 73 years, 55% male) represented multimorbid inpatients with 65 (6.2%) classified as screen-positive by the algorithm. These showed higher expert TTE-measured septal thickness (1.26 [1.07, 1.46] vs. 1.06 [0.9, 1.2] cm; 22% vs. 4% with IVS ≥1.5 cm; P ≤ 0.01). Among 49 (75%) POCUS-flagged positive patients with formal TTE and clinical data, 8 (16%) were confirmed by expert adjudication to have HCM. Specificity is limited by occasional confounding amyloidosis detection (4% of POCUS-flagged patients). This DL-based model identifies HCM and demonstrates feasibility for POCUS screening, supporting earlier detection and broader diagnostic access.

Custom Antibiotic-Coated Intramedullary Nails for Infected Tibial and Femoral Nonunions: Retrospective Cohort Study and Surgical Technique.

To evaluate the safety and effectiveness of a standardized, custom PMMA-coated antibiotic-eluting interlocking nail (Abx-IMN) for chronically infected femoral and tibial nonunion, quantifying infection eradication, radiographic union, and limb preservation. This was a retrospective cohort (single center) with consecutive adults with infected tibial or femur nonunions undergoing limb salvage with a custom PMMA-coated, antibiotic-eluting interlocking nail (Abx-IMN) between 1 January, 2021, and 31 August, 2024. The technique used an ECMO tube mold to cast a uniform ∼1.5 to 2 mm mantle around a standard interlocking nail with 1.5 mm to 2 mm canal over-reaming and protected interlocking windows. The primary end points were radiographic union and limb preservation (analyzed per limb) and infection eradication (analyzed per patient). Proportions were reported with exact 95% confidence intervals. Fourteen limbs in 13 patients were included. Radiographic union occurred in 11/14 limbs (0.78; 95% CI, 0.49 to 0.95). Infection eradication was achieved in 12/13 patients (0.92; 95% CI, 0.64 to 0.99). Limb preservation was maintained in 13/14 limbs (0.93; 95% CI, 0.66 to 0.99). Unplanned revision surgery occurred in 3/14 limbs (0.21; 95% CI, 0.05 to 0.51). The cohort represented a high-risk population with severe soft-tissue injury and a substantial burden of multidrug-resistant organisms. A standardized custom Abx-IMN is a pragmatic, immediately available consolidating strategy for complex infected nonunion, coupling locked intramedullary stability with organism-matched local antibiotics. In a high-risk real-world cohort, it achieved clinically meaningful rates of infection control, union, and limb preservation, with an expected revision surgery burden driven by biology rather than the device. Findings support broader evaluation in multicenter, protocol-driven comparative studies against commercial coatings and frame-based reconstruction.

Anti-Inb alloimmunisation in pregnancy: Anaesthetic and perioperative blood management for caesarean delivery - A case report.

Anti-Inb, a rare high-frequency antigen antibody of the Indian blood group system, renders compatible banked blood virtually unavailable. A 34-week gravida 3 para 2 patient with dual alloimmunisation, anti-Inb (titre 1:128) and anti-D (titre 1:64), underwent elective caesarean delivery. Antenatal iron supplementation raised haemoglobin from 9.6 g/dL to 11.2 g/dL, enabling a preoperative autologous blood donation of 325 mL. The serial middle cerebral artery peak systolic velocity Doppler remained below 1.5 multiples of the median throughout, excluding foetal anaemia. The patient's brother was identified as the sole compatible allogeneic donor. Subarachnoid block with hyperbaric bupivacaine 9 mg achieved T5 blockade. The estimated blood loss was 900 mL due to uterine atony; stepwise uterotonic therapy was employed, and only the autologous unit was transfused. Maternal recovery was uneventful; the neonate required phototherapy but was discharged well. Antenatal haemoglobin optimisation and preoperative autologous donation are essential strategies when compatible blood is unavailable.

Post-Cholecystectomy nutritional management: a comprehensive review of evidence-based guidelines and clinical practice.

Cholecystectomy is one of the most common abdominal surgeries worldwide, yet postoperative nutritional management remains variable. This review synthesizes current evidence on nutritional care after cholecystectomy, with a focus on actionable insights for clinicians. We conducted a non-systematic narrative review of English-language literature published from January 2010 through June 2026, supplemented by manual searches of guidelines. Due to the narrative design, we did not perform a formal risk-of-bias assessment or meta-analysis. Postoperative gastrointestinal symptoms (e.g., bloating, diarrhea, dyspepsia) are common, but their incidence and etiology vary widely. Early oral feeding within 24 h may reduce hospital stay. High-risk patients (diabetes, obesity, elderly) may benefit from protein supplementation (1.2-1.5 g/kg/day) and selective fat-soluble vitamin monitoring when clinically indicated. We provide a risk-stratified algorithm, critically appraise conflicting evidence, and identify knowledge gaps. While standardized ERAS protocols exist for elective cases, complicated scenarios (acute cholecystitis, post-pancreatitis) require special consideration. This review provides an evidence-informed, cautious framework for individualized postoperative nutritional support, emphasizing that most recommendations are extrapolated from other surgical populations and require validation in cholecystectomy-specific trials.

Precise and Rapid Prelithiation of Hard Carbon Anodes via Tuning the Solution Chemistry of Aryl-Lithium Reagents.

While hard carbon (HC) is a compelling anode candidate for lithium-ion batteries (LIBs), its practical viability has long been constrained by the low initial Coulombic efficiency (ICE). Chemical prelithiation using polycyclic aryl-lithium (PAH-Li) reagents offers an effective mitigation pathway, yet current methods struggle with insufficient precision and sluggish reaction kinetics. Here, we report a precise and rapid chemical prelithiation strategy for HC anodes by tuning the solution chemistry of PAH-Li reagents to simultaneously optimize thermodynamic driving force and kinetic pathways of the prelithiation reaction. Through rational molecular design, a customized phenanthrene-lithium/1,2-dimethoxyethane (EΘ Pha-Li/DME = 0.46 V vs. Li+/Li) was identified to precisely match the irreversible lithium-storage threshold potential of HC (EHC-irr = 0.47 V), thereby establishing a self-terminating, spontaneous reaction that delivers an ideal 100% ICE. Furthermore, raising the reagent concentration to 1.5 M progressively drives Pha•- radical anions into the Li+ solvation sheath, weakening the Li+-solvent interaction and lowering the desolvation energy barrier, which accelerates the reaction to reach equilibrium within an unprecedented 90 s. Full cells using the prelithiated anodes demonstrate marked improvements in both ICE and lifespan. This work provides new mechanistic insights into the solution chemistry design of advanced prelithiation reagents for high-performance LIBs.
