Synthesis, Cytotoxicity, and Mechanistic Evaluation of Thiazolium-Based Gold(I) Bis-N-Heterocyclic Carbene Complexes.

Hoffmann Melanie E ME, Haas Lisa C LC, Marques Fernanda F, Sarwary Sarah S et al.

Imidazole- and benzimidazole-based Au(I) N-heterocyclic carbene (NHC) complexes show great potency as anticancer agents. However, other NHC scaffolds are only sparsely explored. Four gold(I) NHC complexes featuring thiazolium-based ligands (NHCthia) with varying N-substituents were synthesized and evaluated for their biological activity. The complexes containing aromatic N-substituents demonstrate higher stability, higher TrxR inhibition, and activity against breast (MDA-MB-231 and MCF-7) and ovarian (A2780) cancer cell lines, compared to the tested alkyl N-substituted complexes. Specifically, mesityl-substituted complex 1 exhibits high activity with IC50 values in the nanomolar range, exceeding the potency of auranofin in MDA-MB-231 cells. Mechanistic studies in leukemia cells (Nalm-6) revealed that complex 1 induces ROS formation, activates BAX and BAK, and causes mitochondrial membrane potential loss, initiating intrinsic apoptosis. Notably, the ligand precursor L1 shows no significant cytotoxicity, confirming that the gold(I) moiety drives the activity. Additionally, complex 1 can overcome resistance to etoposide and prednisolone without affecting healthy leukocytes, making Au(I) bis-NHCthia complexes a compelling class of antiproliferative agents.

Clinical features and outcomes of nocardiosis in Thai patients with childhood-onset systemic lupus erythematosus.

Tangcheewinsirikul Sirikarn S, Nuntasri Sirin S, Rungmaitree Supattra S, Piyaphanee Nuntawan N et al.

Nocardiosis is a rare but life-threatening opportunistic infection in systemic lupus erythematosus (SLE), although data on childhood-onset SLE remain scarce. The objectives of this study were to determine the prevalence, clinical features, associated factors, and outcomes of nocardiosis in cSLE. A retrospective cohort study was conducted in a tertiary academic center including cSLE patients followed between January 2009 and August 2024. Culture-confirmed nocardiosis cases were identified through systematic review of electronic medical records. Firth penalized logistic regression was used to explore baseline factors associated with the development of nocardiosis. Seven out of 323 patients (2.2%) developed culture-confirmed nocardiosis. All were female with a mean age at diagnosis of 11.9 ± 3.1 years. The median (interquartile range) duration from cSLE diagnosis to nocardiosis diagnosis was 4.3 (2.1-13.4) months, with 71.4% occurring within the first year. All patients were receiving corticosteroids at a mean prednisolone-equivalent dose of 1.7 ± 1.3 mg/kg/day at the time of infection. The respiratory tract involvement manifesting as pneumonia was the most common infection site (71.4%). Proliferative lupus nephritis had an exploratory association with nocardiosis (OR 5.03, 95% CI 1.19-28.36, p = 0.028). Rapidly progressive glomerulonephritis at initial SLE diagnosis was significantly more frequent in the nocardiosis group (28.6% vs. 4.4%, p = 0.014). Carbapenem-based induction followed by trimethoprim-sulfamethoxazole (TMP/SMX) consolidation was used in the majority of patients. The in-hospital mortality rate from the index nocardiosis episode was 0%, and the 12-month all-cause mortality rate was 14.3%, attributable to secondary Corynebacterium septicemia in the patient with the most multiple immunosuppressive drug therapy with cumulative burden. Nocardiosis affects 2.2% of cSLE patients and occurs predominantly early in the disease course. Proliferative lupus nephritis had an exploratory association with nocardiosis, highlighting the need for heightened clinical vigilance and the use of antimicrobial susceptibility testing to guide management in high-risk patients.

Persistent Type B Hyperlactataemia as a Diagnostic Clue to Occult Diffuse Large B-cell Lymphoma-Associated Haemophagocytic Lymphohistiocytosis.

Hassan Nasiba N, Khatib Kavish K

Haemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory disorder characterised by excessive immune activation. It may be primary or secondary, with secondary HLH associated with a range of triggers; the most common are infections, autoimmune and autoinflammatory conditions, haematological malignancies and other immune‑dysregulatory states. We present a case of a man in his 70s who presented with a two-week history of exertional dyspnoea, profound fatigue and anorexia. Initial investigations demonstrated marked hyperferritinaemia, thrombocytopenia, mild anaemia, cholestatic liver dysfunction and disproportionately elevated lactate without acidosis. Despite intravenous fluids, lactate remained persistently elevated, consistent with type B hyperlactataemia. Computed tomography (CT) of the abdomen and pelvis demonstrated appearances suggestive of hepatitis but no organomegaly, significant lymphadenopathy or biliary obstruction. Despite empirical antibiotics and fluid resuscitation, he developed progressive pancytopenia, worsening liver dysfunction and rising lactate. Further investigations supported secondary HLH, with an HScore of 186, hypertriglyceridaemia and markedly elevated lactate dehydrogenase. Initial treatment with corticosteroids and anakinra resulted in limited response. An autoimmune trigger was initially considered given his Raynaud's phenomenon and ongoing connective tissue disease investigation. However, bone marrow biopsy subsequently confirmed diffuse large B-cell lymphoma (DLBCL). Reduced-dose rituximab, cyclophosphamide, doxorubicin, vincristine and prednisone/prednisolone (R-mini-CHOP) was commenced, resulting in the resolution of hyperlactataemia, normalisation of blood counts and liver function and improvement in performance status. PET-CT after two cycles demonstrated a complete metabolic response. This case highlights the importance of considering secondary HLH in patients with unexplained cytopenias, hyperferritinaemia, liver dysfunction and persistent hyperlactataemia. Persistent hyperlactataemia despite adequate fluid resuscitation should prompt the consideration of systemic causes beyond tissue hypoperfusion. Bone marrow biopsy is crucial for identifying occult malignancy when conventional imaging fails to show overt malignancy. The early recognition of HLH and investigation for an underlying aetiology may facilitate timely definitive treatment.

Pharmaceutical Residues in Human Urine from Patients Under Antiretroviral Regimens and Self-Reported Healthy Individuals Before and After Three Months' Storage: Implication for Agricultural Reuse in Pretoria, South Africa.

Lizbelt Mugivhisa Liziwe ML, Faith Baloyi Khanyi BK, Oladeji Oluwaseun Mary OM, Olatunji Olatunde Stephen OS et al.

Background/Objectives: Pharmaceutical residues in human urine have raised serious concerns about the safe reuse of urine in agriculture. The current study investigates and compares the concentrations of pharmaceuticals in urine samples collected from male patients undergoing active antiretroviral regimens and self-reported "healthy individuals" at a government hospital in Pretoria, South Africa, and determines how the residues changed during three months of ambient storage, which is the usual recommendation before use in agriculture. Methods: The urine samples were collected in a plastic bottle to prevent fecal contamination, and 15 targeted compounds were analyzed using solid-phase extraction and liquid chromatography-mass spectrometry. The linearity ranged from 0.9910 to 0.9999, 75.5% to 99.1% for recovery, and intra-day and inter-day precision were below 6%. The urine from each group was collected into one pooled sample and then analyzed both pre- and post-storage. Results: Thirteen pharmaceuticals were detected in urine from patients on regular medication before storage; in "healthy individuals", detections rose from eight to twelve after storage. Concentrations in the composite samples after storage were higher for some of the compounds but lower for others, including caffeine, nevirapine, prednisolone and 17α-ethinylestradiol. Metformin was highest overall at 1186.25 µg/L and remained at 806.68 µg/L after storage. The highest concentration for all the pharmaceuticals was reported for metformin, with a value of 1186.25 µg/L, occurring in the group of patients on a regular regimen. There could be a possibility that the rise can be attributed to the hydrolysis of conjugated metabolites during storage; however, no analysis of conjugates was carried out. Conclusions: This study shows the need for appropriate treatment of human urine prior to its reuse as fertilizer and echoes the importance of further research into human exposure pathways and effective pharmaceutical removal strategies.

Low-volume body bag cooling for severe exertional heat stroke in the emergency department: a case report.

Ikeda Takao T, Watase Taketo T, Kondo Takashiro T, Watase Hiroko H et al.

Exertional heat stroke (EHS) is a life-threatening emergency in which delayed or inadequate cooling substantially increases the risk of death and permanent neurological injury. Core temperature reduction to below 39 °C within 30 min is the therapeutic target, and cold-water immersion achieves the fastest cooling rates (0.15-0.35 °C/min) but is frequently unavailable in emergency departments (ED) due to equipment and space limitations. Body bag-based tarp-assisted cooling with oscillation (TACO) has been proposed as an alternative using universally available materials; however, clinical evidence supporting its emergency department use remains limited. A 57-year-old immunocompromised male construction worker (acute myeloid leukemia, status post umbilical cord blood transplantation, maintained on prednisolone and tacrolimus) presented with EHS following 90 min of outdoor labor in ambient conditions of 36 °C and 65% relative humidity. On ED arrival, rectal temperature was 42.0 °C, Glasgow Coma Scale score was 4, blood pressure was 125/80 mmHg, and heart rate was 183 beats/minute. Body bag-based TACO was initiated immediately upon arrival using 15 L of tap water and 15 kg of ice. Endotracheal intubation for airway protection was performed concurrently, demonstrating patient accessibility during cooling. Target temperature (rectal temperature < 39 °C) was achieved in 39 min at a cooling rate of 0.085 °C/min. Glasgow Coma Scale score improved to 14 within 7.5 h of admission. The hospital course was complicated by bacteremia requiring prolonged antimicrobial therapy. The patient was discharged ambulatory on hospital day 42 without neurological sequelae. Body bag-based TACO required substantially less water (15 L) than the water/ice volumes reported in TACO studies (114-151 L) using materials available in any ED. Although the cooling rate achieved (0.085 °C/min) fell below published benchmarks, the patient achieved complete neurological recovery, suggesting that immediate initiation of cooling may help offset a suboptimal rate. While a single case cannot establish equivalence to cold-water immersion, body bag-based TACO offers a practical, low-cost option for initiating cooling when standard infrastructure is unavailable, and merits further comparative evaluation.

Lung Metastases of Pancreatic Adenocarcinoma Masquerading as Temporary Improvement of Bilateral Diffuse Lung Opacities Following Treatment for Interstitial Lung Disease.

Kusunoki Saki S, Tasato Miki M, Sokai Akihiko A, Iwata Toshiyuki T et al.

A 75-year-old man presented with bilateral ground-glass opacities (GGOs) and consolidation on computed tomography (CT). Bronchoalveolar lavage revealed no pathogens or malignant cells. Acute exacerbation of interstitial lung disease was suspected because of rapidly progressive respiratory failure, but steroid pulse therapy was ineffective. After cyclophosphamide treatment, CT showed transient improvement of GGOs and consolidation. Subsequently, worsening bilateral opacities and abnormalities of the main pancreatic duct were detected on CT and magnetic resonance imaging. These radiological findings and sputum cytology indicated lung metastases from pancreatic adenocarcinoma. The autopsy demonstrated lung metastases of pancreatic ductal carcinoma with extensive organizing pneumonia (OP). Although lung metastases from pancreatic cancer are occasional, diffuse bilateral opacities mimicking interstitial lung disease are rare. Improvement after prednisolone and cyclophosphamide may have reflected effects on both OP and metastases. Lung metastases should remain an important differential diagnosis even when bilateral GGOs and consolidation respond to treatment.
