The Association Between Hemoglobin Level and Cognitive Function in Older Adults During Hospitalization: A Prospective Observational Study.

In the ambulatory setting, mildly low hemoglobin (Hb), < 13 g/dL in men or < 12 g/dL in women, is associated with reduced cognition. However, in hospitalized patients, clinicians tolerate much lower Hb levels due to restrictive transfusion policies. Older adults who are at higher risk for cognitive impairment may also be particularly susceptible to impaired cognition from lower Hb levels. The purpose of this study was to test for an association between restrictive Hb levels during hospitalization and patient's cognitive function, particularly in older adults. The data for this study came from an ongoing prospective observational study of hospitalized general medicine patients at the University of Chicago Medical Center (UCMC). Any adult with a Hb < 10 g/dL between March 2017 and February 2020 was eligible. Cognition was measured using the Short Portable Mental Status Questionnaire (SPMSQ), which ranges from 0 to 10. Higher scores indicate worse cognition. Zero-inflated Poisson (ZIP) and logistic regression were used to test the association between cognition as the dependent variable, measured by the SPMSQ, and patients' nadir Hb and age as predictor variables, controlling for race, sex, Charlson Comorbidity Index, transfusion, and a diagnosis of dementia. The SPMSQ questionnaire was completed by 5112 patients who had Hb < 10 g/dL. Lower nadir Hb levels were associated with decreased cognition (higher SPMSQ). Across all ages, a 1 g/dL decrease in Hb was associated with a 7.2% increase in SPMSQ score (95% confidence interval (CI) = 2.9%-11.7%). The effect of decreasing Hb on cognition was greater in older populations, where a 1 g/dL decrease in Hb was associated with an 8.8% (3.1%-14.8%) and 20.8% (9.7%-33.0%) increase in SPMSQ score for patients above 50 and 65, respectively. Lower Hb levels during hospitalization are associated with decreased cognition, especially in older adults.

Haematological Recovery After Discontinuation of Co-Trimoxazole Prophylaxis Among People Living with HIV at an Urban Centre in Uganda: A Retrospective Cohort Study.

Co-trimoxazole (CTX) prophylaxis among people living with HIV (PLWH) significantly reduces HIV-related morbidity and mortality due to bacterial infections and malaria, especially those with advanced HIV disease. CTX prophylaxis is associated with cytopenias, particularly anemia and neutropenia. However, the proportion and factors associated with hematological recovery (resolution of cytopenias) after discontinuation of CTX prophylaxis have not been fully investigated. A retrospective cohort study was conducted using electronic medical records of PLWH with anemia (hemoglobin [Hb]<11g/dl for females and Hb<12g/dl for males) and/or neutropenia (neutrophil count <1×103/µL) before discontinuing CTX prophylaxis, and their CBC reports were reviewed every 3 months after CTX discontinuation. Haematological recovery was defined as Hb recovery as an increase in Hb by >2g/dl or Hb improvement to ≥ 12 g/dl (males) and ≥ 11 g/dl (females), and neutrophil count recovery as an increase in neutrophil counts by ≥ 0.5×103/µL in absolute values or neutrophil count ≥ 1×103/µL after discontinuation of CTX prophylaxis. Multivariable logistic regression models were used to determine factors associated with haematological recovery. The median (IQR) age of the participants was 46 (39-52) years, and the majority were females (68.1%). Of 263 study participants, the proportions of Hb recovery and neutrophil count recovery were 42.9% and 81.1%, respectively. Overweight participants were less likely to experience Hb recovery (AOR=0.33, CI: 0.12-0.93), and males were less likely to have neutrophil count recovery (AOR=0.28, CI: 0.10-0.92). A significant proportion of PLWH who had cytopenias before discontinuation of CTX prophylaxis experienced haematological recovery after discontinuation of CTX prophylaxis. Therefore, discontinuation of CTX prophylaxis may be considered among PLWH during the management of persistent anemia and/or neutropenia.

Efficacy and impact of hypocrellin B-mediated antimicrobial photodynamic therapy against biofilms of Candida albicans.

Candida albicans biofilms pose significant challenges in clinical settings due to their resistance to conventional antifungal treatments and their association with increased morbidity. This study aimed to evaluate the antifungal efficacy of hypocrellin B (HB)-mediated antimicrobial photodynamic therapy (aPDT) against biofilms formed by various strains of C. albicans, including standard, azole-sensitive, and azole-resistant strains. The effects of HB-aPDT on the viability, metabolic activity, and biomass of C. albicans biofilms were assessed using colony-forming unit (CFU) assays, XTT reduction assays, and crystal violet (CV) staining. Confocal laser scanning microscopy (CLSM) was used to observe changes in cell membrane integrity. The generation of reactive oxygen species (ROS) was analyzed using flow cytometry, and the impact on gene expression was examined using quantitative real-time PCR (qRT-PCR). HB-aPDT significantly reduced the survival of C. albicans biofilms in a dose- and light-dependent manner. CLSM revealed photodamage to cell membranes post-treatment, and an increased presence of ROS was observed in the treated biofilms. Gene expression analysis showed downregulation of virulence-related and ergosterol biosynthesis genes, indicating a potential disruption of key pathways in fungal pathogenesis. HB-mediated aPDT effectively reduced the viability and disrupted the structural integrity of C. albicans biofilms, including those resistant to conventional antifungals. This study highlights the potential of HB-aPDT as an innovative approach for managing drug-resistant Candida infections and offers a promising alternative to traditional antifungal therapies.

Untangling the Roots of HPV Vaccine Acceptance in West Virginia: How Hesitancy and Misinformation Shape Parental Decisions.

West Virginia is of particular concern regarding HPV vaccine hesitancy, currently ranking 45th among U.S. states in HPV vaccine uptake. General vaccine hesitancy and HPV- and HPV vaccine-specific misinformation, both associated with lower vaccination, have increased in recent years, complicating efforts to identify key drivers of HPV vaccine acceptance and effective intervention targets. This study used a Qualtrics survey of n = 330 parents of children aged 0-14 in West Virginia. Measures included parental HPV vaccine acceptance for their youngest child (reflecting either completed vaccination or intention to vaccinate), general vaccine hesitancy measured using the Vaccine Hesitancy Scale (VHS), and endorsement of nine common HPV- and HPV vaccine-specific misinformation statements. Misinformation items were summed and demonstrated good internal reliability (Cronbach's α = 0.868). Hierarchical logistic regression analyses indicated significant racial/ethnic differences across all model steps, with non-White parents less likely to report HPV vaccine acceptance for their child (p < 0.001). In Step 2, lower levels of general vaccine hesitancy predicted higher HPV vaccine acceptance (p < 0.001). In the final model, higher endorsement of HPV- and HPV vaccine-specific misinformation was associated with lower HPV vaccine acceptance (p = 0.035). These findings underscore the independent and additive influence of general vaccine hesitancy and HPV- and HPV vaccine-specific misinformation on parental HPV vaccine acceptance in West Virginia. Community-engaged public health strategies should prioritize strengthening vaccine confidence and directly addressing HPV- and HPV vaccine-specific misinformation, particularly among underserved populations.

Human Papillomavirus Vaccine Intent and Associated Factors Among the Unvaccinated University Freshmen in the Largest University Town in China.

Background: Human papillomavirus (HPV) vaccine has been proven a safe, necessary, and effective measure against HPV infection. However, HPV vaccine uptake remains limited among university students in China. This study investigated HPV vaccine intent among university freshmen in the largest university town in China. Methods: A cross-sectional online survey was conducted among unvaccinated female and male freshmen attending seven universities in Songjiang University Town, Shanghai, during 2024-2025. The questionnaire collected sociodemographic characteristics, HPV and HPV-vaccine-related knowledge, awareness, and vaccine intent. Factors associated with HPV vaccine intent were determined. Results: A total of 3397 valid questionnaires were collected, including female (60.02%) and male (39.98%) university freshmen. Overall, 76.12% were aware of HPV, 80.98% were aware of the HPV vaccine, and 84.31% expressed vaccine intent, with significantly higher rates among females than males (each p < 0.001). Socioeconomics, knowledge, awareness, sexual behavior, and HPV testing history were significantly associated with vaccine intent (each p < 0.05). The most common reason for no intent was perceived low risk of HPV-related diseases (40.53%). The most expected improvement measures were regulatory confirmation of vaccine safety and effectiveness (37.71%) and healthcare professionals' recommendations (37.71%), with no gender difference (each p > 0.05). Notably, 4.00% refused HPV vaccination, regardless of improvement measures. Additionally, most respondents preferred financial supporting policies, regardless of gender or vaccine intent (each p > 0.05). Conclusions: University freshmen showed the disparity between high awareness/intent and low knowledge. Financial considerations may influence HPV vaccination decisions. Thus, improving knowledge, particularly among males, and providing financial support may enhance HPV vaccine intent.

[Research progress in combination vaccines containing sabin strain inactivated poliovirus vaccine].

Sabin strain inactivated poliovirus vaccine (sIPV), derived from attenuated Sabin poliovirus strains, has emerged as a promising alternative to both oral poliovirus vaccine (OPV) and conventional inactivated poliovirus vaccine (cIPV). It eliminates the risk of vaccine-associated paralytic poliomyelitis (VAPP) and the emergence of vaccine-derived polioviruses (VDPVs). In addition, sIPV demonstrates superior biosafety and vaccine safety during production and administration compared with cIPV. The integration of sIPV into combination vaccines with other routine immunization antigens can reduce the number of injections and streamline immunization schedules, making sIPV-containing combination vaccines a key focus of recent research. Japan launched the world's first sIPV-containing tetravalent vaccine in 2012, followed by the introduction of an sIPV-containing pentavalent vaccine in 2024. In China, a standalone sIPV was licensed in 2015, and multiple sIPV-containing combination vaccines are currently in clinical trials or preclinical development. The primary challenges for these vaccines include antigen compatibility within combination formulations and the scalability of sIPV production. Future strategies, such as the use of innovative adjuvants and intradermal administration, may enable antigen-sparing approaches. This review provides a comprehensive summary of the development and application of sIPV-containing combination vaccines, aiming to inform and guide ongoing vaccine research and development in China.
